**Position**: GAIN Behavioral Support Ontario RPN (Full-time)

**Hours of Work:**:35 hours per week

**Reports to:**:Director of Care Services

**Deadline**: Until position is filled

**Position Summary**:
The BSO clinician provides support and expertise to patients who are experiencing responsive behaviors related to dementia, complex mental health issues, substance abuse and/or other neurological disorders; and their caregivers/family. In collaboration with the GAIN interprofessional team, the BSO clinician provides a comprehensive, bio-psychosocial behavioral assessment, interventions and follow up. Caregiver education and follow up is a key aspect of this role. The BSO Clinician is considered an expert in the field and a resource for caregivers, the interprofessional team and service providers.

**Responsibilities**:
**1. Provides Clinical Care and Consultation Services to improve and /or maintain activities of daily living for older adults and seniors with responsive behaviour**:

- Participates in GAIN processes including clinical triage and comprehensive Geriatric Assessment
- Provides a comprehensive, bio-psychosocial assessment of patients living in the community in collaboration with the GAIN interprofessional team
- Participates in evidence based assessment using various assessment tools (Behavioural Assessment Tool ( BAT), Cohen-Mansfield Agitation Inventory (CMA), Mini Mental Status Examination ( MMSE ) etc. )

**2. Provide education, community and interprofessional resources to patients, patient’s family, and care providers**:

- Assess need for educational support and training to front-line care givers/family and informal care providers and engage with the available supports as required such as GAIN Clinicians, Psychogeriatric Resource Consultants, Alzheimer’s Society, CCAC case managers, GEM etc.
- Provide linkage to the appropriate services necessary to best serve the patient through knowledge of community and interprofessional resources

**3. Ensures quality of service provision and continuous quality improvements**:

- Participates in the development, implementation, and evaluation of quality management programs/initiatives

**Education**:

- Must be registered with good standing with the College of Nurse of Ontario
- Membership of RPNAO (Registered Practical Nurse Association of Ontario) or RNAO**:(Registered Nurse Association of Ontario)
- Current BCLS (Basic Cardiac Life Support)
- Certificate of completion in PIECES, Montessori Based Dementia Care, U First & GPA (Gentle Persuasive Approach)
- First Aid and CPR certification

**Skills & Experience**:

- Minimum of 3 years of experience with conducting home visits, providing both patient, and caregiver support and perform within the full scope of practice of RPN/RN
- Excellent critical thinking skills for the early detection, intervention, and prevention of responsive behaviors
- Patient assessment skills, clinical knowledge and expertise within the RPN /RN scope of practice related to gerontological mental health, addictions and neurological conditions
- Strong knowledge of best practices for dementia, delirium, depression and other mental health issues and their effect on patients, families and caregivers
- Access to a vehicle daily and valid driver’s license required

**Others**:

- Required to work occasional evening and weekend hours
- Required to work at satellites sites, home visits with clients and caregivers; working alone in the home
- Must have access to a vehicle and a valid driver license
- Vulnerable Sector screening required
- Proof of full COVID-19 Vaccination or Medical Exemption Certificate required
