_**Full-Time 1-year Contract Position with the Possibility of Extension**_

This is a new position that supports the Rapid Intervention and Support Team (RIST). RIST is a partnership between the Hamilton Police Service, the Hamilton Paramedic Service, St. Joseph’s Healthcare, the Canadian Mental Health Association
- Hamilton Branch, Wesley Urban Ministries, the Hamilton Regional Indian Centre, the YMCA, and Interval House. The Rapid Intervention and Support Team is built upon the existing Social Navigator Program which consists of a Program Coordinator, 2 Officers and 2 Paramedics. The addition of the 6 specialized civilian positions working alongside the existing Social Navigator Program creates the new Rapid Intervention and Support Team (RIST). RIST currently operates Monday to Friday with the possibility of extending into evenings and weekends. RIST will be made up of a Program Coordinator, Officers, Paramedics, a housing navigator, an addiction navigator, a youth navigator, a women’s systems navigator, an Indigenous navigator and CMHA Hamilton’s Mental Health Navigator. RIST will focus on supporting those who are homeless, experience mental illness and who struggle with addiction. The goal of the team will be to help marginalized individuals by offering “in the moment” supports, helping them navigate social service and healthcare system and referring them to appropriate services, and offering follow-up supports in order to help improve their quality of life and decrease their negative interaction with police.

**Position Summary**:
The Mental Health Navigator (Case Manager) will:

- Provide crisis intervention, mental health assessments, and short term case management
- Be a primary resource to the team regarding navigation of Hamilton’s adult community mental health and court support services
- Contribute generically and collaboratively to all team services for individuals aged 16 or older who have a serious mental illness or concurrent disorder and are in conflict with the criminal justice system
- Develop goal-focussed “in the moment” treatment plans and determine appropriate referrals

**Qualifications**:

- You are an asset to this role if you have strong teamwork and collaboration skills, and a client-centred approach to your work
- French-speaking is an asset.
