**KEY FUNCTION**:
Collaborate with hospitals, regional partners, primary care providers, specialists, and other care providers across Northeastern Ontario to facilitate the adoption and implementation of a regional scale central intake model. Develop and implement workflows for all modalities and support onboarding to the Wait Time Information System (WTIS), regional credentialing, central intake and eReferral.
**REPORTING**:
Under the general direction of the Director, Coordinated Access and Central Intake.
**DUTIES**:
- Facilitate the successful adoption of health service providers to the eReferral and central intake platforms to support the sending and receiving of referrals for various clinical pathways.
- Lead the development of workflows for the various modalities (i.e. diagnostic imaging, surgical domains, etc.) to support clinical integration.
- Inform of any policy changes required as a result of the implementation of central intake projects within respective region.
- Lead outreach and engagement with all health service providers (i.e. virtual and in-person sessions) to support knowledge sharing and adoption of eReferral and central intake.
- Complete readiness assessments with health service providers and hospitals to on-board to eReferral and central intake.
- Complete monthly progress update reports and identify barriers and threats to project success; develop check and adjust cycles and a review process to formulate recommendations for improvement.
- Coordinate linkages between senders and receivers (e.g. health service providers and the hub hospital(s)) to enable the use of eReferral and central intake while considering provider and patient needs (i.e. cultural awareness and sensitivity to navigating regional access to care).
- Support the successful on-boarding of facilities to the Wait-Time Information System (WTIS) in collaboration with Ontario Health and hospitals across the North East.
- Support the roll-out of regional credentialing for assigned group of hospitals in the North East.
- Assess the impacts of Central intake, eReferral and other coordinated access programs in hospitals in the North East and referring providers.
- Regularly attend and present updates to the working group as part of the governance structure.
- Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in the accreditation process and work to ensure that the program and HSN achieve, maintain and continually improve upon their accredited status.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
**QUALIFICATIONS**
**EDUCATION AND TRAINING**:
- Minimum of a four (4) year Bachelor’s Degree in a health related field, or Health Administration field, from an accredited university or equivalent.
- Current Certificate of Registration in good standing with a regulatory college is preferred.
- Completion of a Change Management Certification program is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
**EXPERIENCE**:
- Minimum of five (5) years’ of clinical experience working in a healthcare organization.
- Minimum of three (3) years’ experience in leading change projects, from a continuous quality improvement perspective.
**KNOWLEDGE/SKILLS/ABILITIES**:
- Demonstrated knowledge of WTIS, regional credentialing, central intake and eReferral use is preferred.
- Demonstrated ability to integrate and manage several related plans to support effective project delivery across a large and diverse portfolio.
- Demonstrated knowledge of Change Management principles.
- Demonstrated ability to proactively resolve scope, cost, and scheduling issues.
- Demonstrated ability to manage cross-functional project teams, and develop and maintain productive working relationships with both internal and external partners.
- Demonstrated excellent presentation skills.
