**Qualifications**:

- Undergraduate degree in a regulated health discipline; registration in good standing with appropriate regulatory college
- Master’s degree in Quality, Risk and Safety or Healthcare Administration or health related degree with comparable experience
- 2-5 years clinical experience
- 3-5 years’ experience in quality improvement and risk management within a health care setting
- Certificate in Risk Management preferred
- Lean Six Sigma Green or Black belt preferred
- Strong analytical, negotiation, decision making, problem solving, organizational and presentation skills. Must be a proven self-starter with strong multi-tasking and relationship management skills.
- Strong communication skills - oral, written and listening skills.
- Able to work both independently and as a member of a team and have the ability to interact with diverse groups of individuals.
- Detailed oriented with a high level of accuracy, and provide follow-through on projects.
- Able work in a fast pace and rapidly changing environment.
- Demonstrated intermediate proficiency with MS Suite software
- Expertise with Accreditation Canada’s Programs preferred
- Evidence of satisfactory attendance record
- Ability to work in a manner that exemplifies the centre’s core values and helps patients get their life back

**Accountabilities**:
**General Quality, Risk and Safety**
- Monitors and reports risk exposure and provides guidance to director and other management on risk triggers, limits and actions.
- Works with teams to ensure effective patient safety, quality improvement and risk management strategies arein place.
- Provides an independent review and evaluation to ensure that risk management issues/concerns are being appropriately evaluated, investigated and resolved and/or escalated as indicated.
- Works with the various teams to review and improve policies, best practices and governance.

**Claims Management**
- Works with patient relations, physicians and other personnel to resolve patient safety events.
- Gathers information and documents required for responses to discovery requests

**Investigation and Analysis**
- Investigates reported occurrences and develops strategies for management of patient safety events.
- Researches medical records, policies and procedures to gather information required for further analysis.
- Serves as facilitator for root cause analysis meetings conducted in response to serious safety incidents.
- Monitors compliance with action plans in collaboration with Quality/ Department leaders. Analyzes individual occurrence reports and trends to identify opportunities to improve the delivery of safe patient care. Recommends changes in policies, practice and procedures to prevent patient injury.
- Prepares and maintains summaries of investigations, recommendations and corrective action plans in an event reporting database.
- Coordinates QCIPA investigations in collaboration with Director
- Participates in appropriate committees and meetings as Risk Management representative.

**Education and Guidance**
- Provides quality improvement, patient safety and risk management advice and guidance.
- Assists with the coordination and communication of various functions involving the Enterprise such as correspondence, meetings, presentations, reports, and internal audits.
- Identifies product and regulatory filing issues and provides input to Director for resolution.
- Provide leadership in program evaluation, program development and research as appropriate.
- Participate on internal/external committees.
- Works with Director and team to develop short and long-term strategy, goals, and objectives for the department and assists in the training, development, and motivation of staff to meet or exceed Enterprise goals and objectives.

**Accreditation**
- Coordinates the Centre’s participation in Accreditation Canada’s Qmentum and Stroke Distinction programs
- Acts as primary liaison with Accreditation Canada, submits relevant timely information and keeps abreast of new developments
- Develops project timeline and communication plan
- Coordinates a centre-wide launch and staff survey process
- Provides centre-wide education and communication; supports accreditation teams and coordinates survey completion

**Safety**
- Oversight, development and administrative maintenance of online safety event reporting software (RL6)
