Health Sciences North (HSN) is seeking an innovative leader for the position of **Supervisor Access and Flow**. In this role, you will plan, organize, direct, and evaluate the operation of patients in and out of HSN acute care beds daily, identifying patient flow delays and opportunities, working with Clinical Managers, Clinical Leaders, and Charge Nurses to eliminate delays.

**DEPARTMENT OVERVIEW**
- The Access & Flow department_ is a diverse team with the primary goal of improving patient flow within the organization by addressing challenges like emergency department overcrowding and ensuring timely discharges. Using patient flow data, the department identifies variations in demand and barriers to prompt service access and taking a coordinated approach to guide patients through the healthcare system. The team actively monitors admissions, seeking appropriate inpatient care locations, and collaborates with external hospitals and care facilities to assess occupancy and transfer opportunities. Responsibilities include monitoring out-of-town patients for repatriation opportunities, tracking admission requests, and resolving complex patient placement cases._

**KEY FUNCTION**:
Provide support to the flow of patients in and out of HSN beds, and provide weekend management support on other
issues as required.

**REPORTING**:
Under the general direction of the Manager, System Transformation.

**DUTIES**:
1. Plan, organize, direct, and evaluate the operation of the Bed Allocation department.
2. Monitor the flow of patients in and out of HSN beds daily, and oversee the appropriate placement of patients.
3. Identify patient flow delays and opportunities, and work with Clinical Managers, Clinical Leaders, and Charge Nurses to eliminate delays.
4. Chair the daily morning bed report and work with various programs to develop a bed plan.
5. Work with inpatient departments to establish individual plans for patients that need to be transferred within HSN or to external facilities for treatment.
6. Oversee the use of HSN’s non-urgent patient transportation services daily.
7. Communicate daily with the City of Sudbury’s Emergency Medical Services (EMS) Platoon Chief and Central Ambulance Communications Centre (CCAC) dispatches pertaining to HSN calls.
