**Supervisor, Personal Support Services**

**(Temporary Full-Time - 6-Month Contract)**

At Community Care Durham (CCD), coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people wherever they choose to call home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.

Reporting to the Manager, Assisted Living Services, Supportive Housing, and Respite, the Supervisor, Personal Support Services (PSS) is responsible for a team of Personal Support Workers (PSWs), and Care Coordinators. The Supervisor, PSS oversees the operations and provision of clients of the Personal Support Services team. The Supervisor, PSS provides effective leadership to the program staff to ensure that CCD policies and procedures are met.

Responsibilities include but not limited to:

- Manages a team of Personal Support Workers, Homemakers, and Care Coordinators;
- Conducts supervisory visits for clients, document, and report findings;
- Identifies and manages risk as it relates to vulnerable clients and the local services;
- Conducts client visits and develop care plans in conjunction with the Care Coordinators for new and existing clients as needed;
- Supports team building and manages conflict resolution amongst CCD Workforce;
- Assists in reducing the client waitlist by working with the Care Coordinator and scheduling team under the manager’s direction;
- Monitors targets, plans and implements practices alongside the manager to ensure MSSA targets achieved;
- Prepares monthly management reports to be submitted to the manager to improve efficiencies and processes;
- Participates in the recruitment cycle and on-boarding process of assigned programs in consultation with Human Capital;
- Provides on-going training and support to staff as needed in protocols and practices;
- Collaborates with Management team to improve processes, problem solve issues, develop best practices, create synergies and ensure a holistic client experience;
- Provides leadership and support to staff as part of Manager On-Call rotation (weekends and statutory holidays as needed) and communicates incidents/situations with appropriate Manager/staff as needed;
**Qualifications required**:

- Post-secondary education degree or diploma in Community Social Services or Healthcare related field required
- Nursing or other health care designation is an asset
- Registration in good standing with a Regulated body is an asset
- Minimum three (3) years' experience in a supervisory role
- Strong knowledge of the principles and practices of personal support services
- Experience in client assessment, PSW supervision and Alayacare (or similar) software required
- Intermediate level computer skills required (MS Office and Internet)
- Experience working in non-profit and community care environments is an asset
- Ability to work in a safe manner and promote the health and safety of all staff, volunteers, clients, and the community as outlined within CCD policies
- Ability to travel efficiently within Durham Region with access to a safe and reliable vehicle
- Sensitivity to the needs of the vulnerable population with disabilities, including challenges in cognition
- Satisfactory Police Vulnerable Sector Check
- Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

**Compensation**:Starting at $56,000 annually; three (3) weeks’ vacation per year of work; 9 paid (including stat) holidays; health, insurance, and pension benefits when eligible.
- Pay will be commensurate based on education and experience

**Job Types**: Full-time, Fixed term contract
