As one of 2 BSO Leads, you will providing guidance and support our team members by leveraging your knowledge and skills of best practices in providing non-pharmacological methods to support our residents.

In a collaborative approach the BSO Lead will oversee the coaching, mentoring and modeling strategies that are to be implemented as well as other relevant interventions to manage responsive behaviours. By ensuring a commitment to client centered resident care, the lead will foster a culture that is positive, creative, respectful, flexible, and enthusiastic.

**Responsibilities**

**BEHAVIOURAL SUPPORT**
- Works collaboratively with care partners and consultants serving HMLTCH such as, Alzheimer Society, PRCs (Psychogeriatric Resource Consultants), Specialized Geriatric Services, Community BSO services, BSO Mobile Team.
- Assist residents in dealing with personal and social problems by delivering counseling, connection with community services and social support as well as counseling for daily/ social skills.
- Completes required assessments and the development of strategies or recommendations for the targeted risk population.
- Participates in Continuous Quality Improvement.
- Supports the residents/their caregivers by providing counselling, group or individual according to the College of Nurses of Ontario (CNO) scope of practice.
- Provides recommendations on bio-psychosocial and non-pharmacological interventions to support resident goals related to psychosocial issues.
- Responsible for initial PIECES and other relevant assessments on all new referrals within assigned hours.
- Ensures care strategies progress and are regularly evaluated and adapted in consultation with the clinical care team.
- Utilizes and completes approved assessment tools as per established BSO practice and scope of practice.
- Identify themes and opportunities for educational in-services in collaboration with the multidisciplinary team, other service providers, outreach services/teams and experts in the field.
- Utilizes therapeutic communication by establishing a therapeutic relationship with clients, families, co-workers and others for the purpose of meeting client needs
- Collaborates with the clinical team, families, and service providers in a professional, courteous manner and keeps the BSO Mobile Team Lead informed of any changes or concerns identified.
- Provides support to the care team by trialing, modelling and coaching strategies to mitigate escalating responsive behaviours and Behavioural and Psychological Symptoms of Dementia where appropriate and in support of the reason for referral.
- Participates in ongoing education and training related to best practices and responsive behaviors to the target population.
- Attend Family Council meetings as invited by the Council to explain the role and provide education.
- Leads responsive behaviour meetings and other meetings as requested by the interdisciplinary team.
- As a team, support successful resident transitions by facilitating implementation of care plan recommendations.
- Identify opportunities to build capacity of the team through mentorship and coaching.
- Respond in emergency situations and episodes of responsive behaviour.
- Carry out all duties with appropriate attention for resident/client safety.
- Responsible for ensuring privacy and confidentiality of personal health information in keeping with wishes or consent and capacity of the resident.
- Maintains public health information according to college practices and applicable laws

**OPERATIONAL EXCELLENCE**
- Ensure appropriate screening/ risk assessments are completed for residents with new, worsening or change in behaviours
- Coordinate referrals and engage community partners in supporting the resident care needs with responsive behaviours.
- Facilitate education sessions to build capacity in team members.
- Supports the management of case reviews, clinical rounds and engage the interprofessional team in the development of an individualized plan of care to meet resident needs.
- Attend and support during resident care conferences - Assist with Hight Intensity Needs (HIN) Fund supplementary staffing submissions. - Performing other duties as assigned

Pay: Up to $65,000.00 per year
