**Job Overview**:
We are looking for compassionate and skilled Registered Practical Nurses to provide high-quality care to our clients. If you have a passion for helping others and making a difference in people's lives, this role is for you.

**Responsibilities**:

- Develop and modify care plans in response to residents' changing status
- Liaise with other healthcare personnel, agencies, and families
- Maintain good relationships with medical/support staff, residents, families, and the general public
- Supervise unit activities and take charge as required
- Monitor performance and complete evaluations for Personal Support Workers (PSWs)
- Direct, assign, and evaluate the work of PSWs
- Utilize leadership skills to identify and solve problems on the unit
- Ensure care delivery is consistent with the resident Bill of Rights
- Understand and work towards improving quality indicators used by the facility
- Manage nursing supplies and equipment efficiently
- Conduct unit rounds at the beginning and end of shifts
- Administer medications and treatments in compliance with pharmacy provider policies, facility policies, and College of Nurses standards
- Maintain accurate and complete records of nursing observations and care
- Assess changes in health status and report to Substitute Decision Makers (SDMs) and the interdisciplinary team

**Qualifications**:

- Valid Registered Practical Nurse license in Ontario
- Strong clinical skills and knowledge of long-term care and/or home care environments
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Leadership and problem-solving skills
- Commitment to ongoing professional development
