Diabetes Educator

Our Diabetes Educators work as a team providing support, guidance and education to patients within the Family Health
Team. This position reports to the Clinical Manager.
The main responsibilities for this position are:
are aligned with professional governing body, and that are aligned with the overall objectives of the BCFHT**
- Provide in-depth assessments to patients with diabetes to establish and identify the priorities for the patient’s
care**
- Work with patients to develop a care plan that encourages and supports self-management, and that sets goals in
order to achieve desired health outcomes**
- Develop and deliver education aligned with patient’s individual learning styles, and with the patient’s readiness
to move forward in the self-management of their care**
- Triage referrals that are received by the Chronic Disease Prevention & Management (CDPM) clinic to prioritize
their level of urgency**
- Conduct follow-up appointments with patients, either in person or by telephone**
Ensure quality and integrity of all patient information through the accurate use of electronic medical record(EMR)** Provide detailed reports to patient’s family physician through the EMR** Collaborate with the Manager, CDPM in the continued development and revision of the diabetes program to
meet changing needs** Continually develop educational resources and self-management tools that are aligned with best-practice
standards, and that will enhance and support the patient’s learning objectives** Collaborate with Administrative Assistants to ensure the scheduling of patients is efficient and is aligned with
the objectives of the program** Collaborate with other CDPM team members as the diabetes program evolves to ensure continuity and
efficiency of care** Collaborate with community partners to provide seamless transfer of information between the acute and
community sectors** Provide education and guidance to other BCFHT team members pertaining to diabetes**
**Education, Qualification and Skills**
- A Bachelors Degree in Nursing plus 5 years experience in a nursing role**
Current registration with the College of Nurses of Ontario** Certified Diabetes Educator, or working towards, considered an asset** Demonstrated experience educating and working with patients who have diabetes** Experience in working within a diabetes program** Strong understanding of current clinical practice guidelines related to diabetes** Experience using an electronic medical record (Accuro)** Strong working knowledge of Microsoft Office including Word, Excel and Outlook** Excellent communication and organizational skills** Must be able to work effectively with a team of integrated health care professionals to provide collaborativeteam based care** It is a condition of employment that you receive the necessary vaccinations to protect against COVID-19, or any

will comply with the Ontario Human Rights Code with respect to any exemption requests
**Additional Job Requirements**:
- Must adhere to all BCFHT Policies and Procedures
- Must be legally entitled to work in Canada
- Must be able to travel within the community
