**Position Summary**:
The Program Support Worker provides support, assistance and/or resources to individuals and the collective on food security issues. The Program Support Worker works closely with community partners, individuals, families and groups to establish and strengthen food security programs at Hamilton Urban Core and improve the capacity of participants to improve their own health, well-being and access to appropriate services and resources. Meetings with individual and group services and outreach efforts may be provided at the Centre, or in the community. The Program Support Worker also supports with the creation of flyers and education for the Community Health Promotion Programs.

**Key Responsibilities**:
Responsibilities are divided into two primary categories:
**Food Security (60%)**
- Support and coordinate food security initiatives establishing terms of reference, identifying priority populations, and presenting actionable plans to address food access challenges, for vulnerable populations.
- Plan, implement, and evaluate food security programs, establishing terms of reference, identifying priority populations, and presenting actionable plans to address food access challenges.
- Collaborate with community partners to develop and implement food distribution programs.
- Assist in organizing and promoting food drives and related community outreach.

**Community Health Program Flyers (40%)**
- Support with the design and creation of community health programs flyers and educational materials that inform clients about available services.
- Work with program teams to ensure materials are accessible, culturally appropriate, and align with the Centre’s health equity approach.

**Other**:

- Maintain accurate, current client records, data and information and electronically encounter client services in accordance with the Centre’s policies, best practices and standards of good practice. Complete and submit grants and reports in completing reports as required and in accordance with the timelines and standards established by the Centre.
- Conduct and facilitate evaluation of activities or programs in accordance with the Centre’s evaluation framework and make recommendations for continuous improvement.
- Assist in the continuing education of staff and volunteers regarding knowledge and understanding of issues related to food security.
- Actively participate in the Centre’s quality improvement and quality assurance plans. Participate in meetings of the Centre as appropriate such as team meetings, case conferences and staff meetings.
- Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning.
- Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses or educational leave).
- Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework.
- Work as part of the interprofessional team at the Centre and adhere to the Centre’s core competencies and the CHC Model of Care.
- Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core.

**Performance Targets**:
The approximate number of clients served annually is 300 unique individuals.

**Qualifications**:

- Demonstrated experience working with marginalized individuals and communities.
- Undergraduate degree in relevant discipline from a recognized university or combination of some post-secondary education and significant experience in food security, and/or community development.
- Demonstrated experience of working within an anti-racist, anti-oppressive, anti-discrimination, cultural sensitivity/appropriate framework.
- Working knowledge of a wide variety of community resources, service providers and agencies.
- Demonstrated experience in developing outreach strategies and conducting outreach activities.
- Ability to work independently and within a multi-disciplinary team.
- Knowledge and understanding of community health Centers
