**JOB SUMMARY**
This position is responsible for advancing the integration and coordination of a mental health and addictions system in the Niagara Region along a continuum of care that includes acute, community mental health and addiction services and primary care. This position will provide leadership, direction, coordination and management in the development and implementation of a model of care that includes a process that streamlines access to phases of care, where patients can move up to a higher level of care or step down to a lower level of care depending on their care needs. As the Project Manager, you will work with a variety of community partners including other staff within the Niagara Ontario Health Team-ESON (N-OHT-ESON), people with lived experience and allied service providers, the Mental Health and Addictions Working Group of the N-OHT-ESON and the OHT-ESON Psychiatrist LEAD along with hospitals, mental health and addictions service providers and primary care.

**QUALIFICATIONS**

**EDUCATION**
- A university degree.
- Masters in a Health discipline Preferred.

**EXPERIENCE**
- Minimum 5 years of demonstrable experience as a professional project manager from initiation to completion.
- Three years in project management theory and processes, including planning tasks and allocating resources, risk management, issues management, time management, financial management, change management, working in teams, quality management, monitoring and reporting, documentation and record keeping.
- Experience in healthcare environment is preferred.

**QUALIFICATIONS, CERTIFICATIONS, OR DESIGNATIONS**
- Project Management Professional designation and/or trained in the principles of Lean Six Sigma Improvement methodology and certified at the Black Belt level.

**RESPONSIBILITIES**
- 1. Develop detailed project plan (50% of the time):
- Develop and maintain an updated master project plan as well as sub-project plans for monitoring and tracing deliverables.
- Identify resource availability for specific projects/tasks and make recommendations for resource reallocation.

2. Communication (30% of the time):
- Communicate and disseminate plan(s) to appropriate individuals to ensure each person understands the overall scope of the project and when tasks are initiated and completed.
- Support the preparation of progress reports, standardized reporting procedure and the monitoring of overall project performance.
- Develop briefing notes, presentations, and other key project documents to support communication and reporting.

3. Risk Assessments (10% of the time):
- Complete risk assessments; provide inputs and analysis for prediction of project and resource risk.

4. Liaison (5% of the time):
- Act as a liaison and source of information for internal and external stakeholders.

5. Other (5% of the time):
- Support other analysis as required.

Perform other related duties and responsibilities as assigned or required.

**SPECIAL CONDITIONS**
- Shared reporting relationship between the Director of Clinical Services at Niagara Region Public Health and the N-OHT-ESON.
- Must possess and maintain a valid and unrestricted specified license for the class of the vehicle operated.
