**Job Summary**:
Reporting to the Manager Homeless Operations, the Recuperative Care Supervisor, will liaise with the hospital to assist individuals who are homeless with the supports they require to recuperate and successfully transition back into the community after discharge from hospital. This position will work closely with the Niagara Health System to ensure individuals meet the eligibility criteria of entering into this program. The Supervisor is responsible for managing the recuperative care unit in Bridge Housing while providing safe, client focused, and quality care. They will perform and collaborate as a member of the interdisciplinary team which includes the Recuperative Care Support Worker, Intensive Case Manager, Bridge Housing Shelter Supervisor, Homelessness Operations Manager and Medical Director.

**Education**:

- Bachelor’s degree in a healthcare related field with active registration / membership in a health or health related professional college.
- Regulated Healthcare Professional with current registration with a relevant college and member in good standing (e.g.) RN, RPN, OT, Social Work etc.

**Knowledge**:

- At least 3 years experience working with marginalized peoples and/or vulnerable population.
- At least 3 years of current related experience in a leadership position.
- At least 3 years experience in a human services and/or public sector environment, working with multiple social/community agencies.
- Dynamic interpersonal and leadership skills including critical thinking, communication, responsiveness, conflict resolution and problem solving.
- Experience supervising or managing shelter and/or hostel operations an asset.
- Experience in collaborative care planning within hospital and community setting an asset.
- Knowledge of relevant Acts and Regulations.

**Responsibilities**:
**_ Admission and Discharge of Recuperative Care program. (30% of time)._**:

- Collaborate with Niagara Health System on discharge of homeless individuals from Hospital to Recuperative Beds.
- Supports/coordinates client discharge from hospital to program ensuring client needs are consistent with program resources and suitability criteria.
- Facilitate all aspects of admission process.
- Document all client admissions as per program requirements.
- Review care plans as outlined by discharge planners at the hospital and identify the clients who require a short-term recuperation as outlined in the admission eligibility.
- Manage and maintain waitlist in consultation with the hospital discharge planner and Medical Director.
- Successfully transition individuals back into community after discharge from program in collaboration with Medical Director and Bridge Housing Shelter Manager.
