Mental Health Facilitator

**Mental Health Facilitator - Full-Time (1.0) EFT - Indefinite Term**
A professional independent practitioner, fully accountable and responsible for their own practice, working within a multi-disciplinary team, works with individuals who access community health centre programs and services.

The Mental Health Facilitator is an advanced facilitator role supporting direct service delivery, program development and evaluation. This role reports directly to the Program Manager and the general administrative direction of the Executive Director and is accountable to the communities we serve.

**KEY RESPONSIBILITIES**

Mental Health/Wellbeing Programming and Supports:

- Provide mental health/wellbeing services and supports within the scope of relevant professional standards including: assessment, screening/intake, health education, group facilitation, referral and follow-up, outreach and other relevant activities.
- Consult with other interdisciplinary providers and make appropriate referrals when the person’s situation requires supports beyond the scope of practice.
- Guide people we serve through coping exercises including mindfulness, grounding, self-compassion all from a trauma-informed, culturally safe and harm reduction foundations.
- Provide coaching, counseling and direct treatment within scope.

Crisis-Intervention and De-Escalation:

- Evaluate immediate safety risks, de-escalate and diffuse conflict in an assertive, timely fashion
- Integrate trauma-informed/healing centered principles and strategies in preventing and diffusing situations
- Create and facilitate a calm, compassionate environment for those experiencing distress.
- Ethically navigate in a trauma informed client centered way client situations regarding Professional duty to report (Suicidal ideation, homicidal ideation, harm to a child etc).

Health Education & Management:

- Provide the people we serve with education/resources related to health practices, illness prevention and navigation of the health care system to access health services.
- Complete intakes, assessments and coordinate service delivery plans as appropriate and within your scope
- Facilitate health education programs for individuals/families in the communities we serve.

Program Administration:

- Responsible for various day to day oversight of your program activities within a program/service area as directed by the Program Manager
- Compile informational and statistical reports as required
- Maintain complete and accurate charts and medical records, and participate in chart reviews and case conferences.

Group Facilitation:

- Provide safe, critical, competent, ethical, compassionate, and evidence-informed care as it relates to mental health and wellbeing, across the lifespan in response to community and individual needs.
- Provides assessment, monitoring and education to support community members in and improving their well-being.
- Practice from person-centered, harm reduction, social justice and Trauma informed/healing centered approaches.
- Provide effective management of care (comprehensive and systematic assessment, critical thinking, problem solving and clinical reasoning in decision-making.

Advocacy:

- Support community members to voice their needs to achieve optimal health outcomes.
- Support community members who may be unable to advocate for themselves.
- Advocate for process/policy change to promote safety, equity, access, person-centered, trauma informed and harm reduction approaches.
- Work to eliminate healthcare disparities in priority populations.
- Advocate for the use of Indigenous health knowledge and healing practices in collaboration with Indigenous healers and Elders consistent with the Calls to Action of the Truth and Reconciliation Commission of Canada.
- Advocate for the use of culturally safe approaches in all aspects of health promotion and treatment.
- Advocate for health equity approaches.

Educator/Facilitator:

- Supports and demonstrates respect for choices made by community members about their own learning, goal, pace, and outcomes.

**Qualifications**:

- Relevant combination of education and experience in the areas of social services, mental health or related field.
- Minimum 2 years’ of group facilitation in a community-based setting
- Minimum 2 years’ experience in mental health promotion and intervention
- Knowledge of Adult Learning and Behavior Change Principles
- Experience working with youth, young adults and seniors an asset
- Fluency in French preferred
- Knowledge and understanding of diverse traditions and cultural practices with an emphasis on comfort and understanding of Indigenous practices.
