**Respite Facilitator** to join our team.

The position of Intake/Resource Facilitator acts as a first point of contact for all outside correspondence for individuals, families and community stake holders regarding services and programs provided by VITA or the Developmental Services Sector.

This position facilitates services for adults with developmental disabilities and their families and supports community integration by researching, advocating, and coordinating available services within VITA and connecting the individuals and their families to the resources outside of VITA.

Focus of the position is to gather all pertinent information necessary to access VITA’s funded and fee for service programs and services, complete and organize appropriate assessments to enable the individuals with developmental disabilities to have the optimal experience in participating in desired programs and services.
- This position is also responsible to fill and identify the vacancies in all programs and services that VITA offers.

**JOB RESPONSABILITIES**
- Relationship management and providing service options to adults with developmental disabilities and their families. Complete the intake function for all members who are accessing VITA services and support
- Support community participation by, researching, advocating and coordinating supports to promote full community integration.
- Clear understanding of local access mechanisms and VITA’s role.
- Establishes funding sources within and outside the developmental sector.
- Participate as members of Community Committees representing VITA with other community support agencies and Ministry representatives
- Work closely with Program staff to support goals identified in the individualized plans.
- Performs other related duties assigned by the Manager or delegate

**WHAT YOU BRING**:

- College or University Degree in a related field or other relevant work experience
- Must have strong administration and computer skills for service data management
- Comfortable with new technologies to integrate into individualized service
- Knowledge of developmental services sector in relation to access, and distribution of resources
- Experience working with marginalized individuals, individuals with disabilities and families
- Exceptional communication and presentation skills
- Valid driver’s license required

**Shift: Monday to Friday 9am-5pm, occasional Saturdays as required**

**Employment with VITA is conditional on your ability to meet the following requirements**:

- Proof of negative COVID-19 test completed in the last 3 days before start date.
- Proof of completed COVID-19 Vaccinations, or a commitment to receive the second dose in the first week from the date of hire (or documentation proving inability to receive the Vaccine due to medical or religious grounds).
