**Job Overview**
We are seeking a compassionate and dedicated Support Worker to join our team. In this role, you will provide essential assistance to individuals in need of support, ensuring their comfort and well-being. Your responsibilities will include aiding with daily living activities, promoting independence, and delivering high-quality care tailored to each individual's needs. This position is ideal for those who are passionate about making a difference in the lives of others.

**Responsibilities**
- Assist clients with daily living activities such as bathing, dressing, and grooming.
- Provide meal preparation and ensure proper nutrition for clients.
- Offer patient care, including companionship and emotional support.
- Administer medication as prescribed and monitor clients’ health conditions.
- Deliver specialized care for individuals with dementia or other cognitive impairments.
- Support individuals with disabilities in achieving their personal goals.
- Maintain a clean and safe environment for clients, ensuring hygiene standards are met.
- Collaborate with healthcare professionals to create and implement care plans.
- Document care provided and report any changes in client condition to supervisors.

**Requirements**:

- Previous experience in assisted living, home care, or caregiving is preferred.
- Knowledge of dementia care and hospice care practices is a plus.
- Ability to perform medication administration safely and effectively.
- Strong communication skills to interact with clients, families, and healthcare teams.
- Compassionate demeanor with a genuine desire to help others.
- Physical capability to assist clients with mobility and personal care tasks.
- Must be reliable, punctual, and able to work flexible hours as needed.
