They will:

- Lead the implementation of the palliative models of care (MOC) implementation measurement and evaluation plans (including measurement and evaluation plans for Indigenous-focused initiatives), along with the development of the provincial measurement strategy for palliative care
- Lead monitoring and evaluation of the palliative MOC implementation in identified implementing organizations to ensure the models address barriers to integrated palliative care for individuals and their families and caregivers and improve quality and outcomes.
- Collaborate with internal and external partners to ensure measurement and reporting on palliative care aligns with palliative MOC recommendations and expected OH reporting standards
- Where Indigenous communities wish to be involved, provide support to Indigenous communities and Indigenous organizations for their measurement efforts in partnership with OH Regions

Here is what you will be doing:- Responsible for workstreams and deliverables of a high priority, complex, long-term duration project. Manages multiple operational tasks concurrently.- Guide project staff in the achievement of deliverables.- Conduct regularly scheduled status reviews, provides business guidance, proactively identifies deviations from the work plan and implements corrective action.- Develop and present material for review and informational purposes to express complex concepts effectively.-
- Identify improvement opportunities and escalate key issues/risks to the Group Manager; proposes and executes solutions to address challenges or mitigate risks- Support teams to ensure health service provider and service provider organization adherence to health service accountability agreements.- Coordinate and conduct analysis of local health-care system based on provincial data, community input and other sources for purpose of identifying gaps and establishing planning priorities.- Participate in and contribute to provincial planning activities and conducts effective partner consultation related to assessing impact of planning recommendations and decisions and promoting consensus among partners for action required

Here is what you will need to be successful:
Education and Experience- A Master’s Degree in Health Science, Health Administration, Public Health, Business Administration or Health Informatics or related discipline or equivalent work experience, is required.- 6-8 years of overall work experience with performance measurement and evaluation in health care is required, ideally in palliative care- 2 years of experience implementing healthcare projects and/or 2 years clinical quality improvement experience preferred- Experience in change management, process development and project management operations is required.- Demonstrated experience with palliative care projects required.- Experience with community health organizations or long-term care homes is an asset.

Knowledge and Skills- A commitment to developing a coordinated and standardized approach for delivering palliative care services for all people in Ontario, regardless of their age or illness- A strong understanding of Ontario’s health care system, key provincial priorities for palliative care and broader health system improvement as well-
- Extensive/Advanced expertise in conducting qualitative analysis using software such as NVivo.- Extensive expertise using data from qualitative and quantitative analyses and other sources to advance QI, including leading completion of literature reviews, jurisdictional scans and other data collection activities for planning, monitoring and evaluation purposes-
