**Outreach Facilitator (Full-Time) - Richmond, BC**

As a key member of the Outreach team you will provide services to individuals to live in their own community, with needed supports to ensure long term success. The services are based on the specific needs of each of the individuals supported and you will work to create a clear and strong natural support system.

**YOUR IMPACT**:

- Support individuals to plan activities that meaningfully engage and expand their social networks, community connections, and life experiences.
- Build support networks in the community consisting of but not limited to, professionals, family members, friends and organizations to ensure services are in the client’s best interest and supportive of their goals and vision.
- Provide targeted and meaningful supports that enrich each individuals’ chosen lifestyle, with the goal of fading out if and as appropriate.
- Support independent decision making that is safe and respects dignity of risk.
- Be a leader in facilitating self-advocacy.

**YOUR CAPABILITIES**:

- Minimum two years’ experience in a community-based setting with an emphasis in community outreach and working with high risk/vulnerable populations.
- Excellent verbal and written communication, as well as conflict resolution skills.
- Strong planning skills in areas of life-skills, recreation and educational opportunities
- Flexible and able to work independently and as part of team that values communication, integrity and, diversity.
- Respectful and sensitive to people from diverse cultural, health and socio-economic backgrounds.
- Advanced computer skills with Microsoft Programs (Word, Excel, etc.), experience with Sharevision would be an asset
- Current CPR and First Aid Certification and Clear Criminal Records Check
- Valid BC Driver’s License and access to a vehicle.

**Salary**: $50,000.00-$52,000.00 per year
