**MSW Position Profile**

v **East GTA Family Health Team (FHT) provides primary health care to residents living in the Scarborough and surrounding areas. It offers various chronic disease management and preventative / educational programs. These programs are available to both patients rostered with some 26 family physicians affiliated with FHT as well as another 50 other family physicians in the region.**

v **Our mental health program is one of the most sought out programs, and the team consists of five MSWs and a psychiatrist affiliated with our FHT. The team is also supported by our Lead Physician.**

v **The program includes a comprehensive intake assessment, developed with the help of a psychiatrist, followed by one-on-one and/or group sessions using CBT, DBT, Mindfulness

v **We use Accuro (EMR) to document patients’ journey, and communicate with referring family physicians.**

v **FHT is funded exclusively by the province (MOH).**

v **We offer a flexible signature benefit plan to suit each individual situation, and a flexible work arrangement to support work-life balance. There is also free parking.**

v **FHTs report program outcomes to the funder on a quarterly basis.**

v **Compensation - the gross annual salary is up to approximately $74 K; in addition, we also offer a flexible benefit plan --- this amounts to an additional approximately 20% of gross salary.**
