As a member of the Adult Day and Caregiver Support team, the Intake Coordinator is responsible for conducting client initial assessments for all Acclaim Health Adult Day Programs and Overnight Respite Program, JIM’s Suites. This is a full time, 1 year contract position where the incumbent must be able to work throughout the Halton Region and Mississauga and have a flexible schedule to include working from home when required. This position is Monday to Friday from 8:30am - 5:00pm.

**PRIMARY FUNCTIONS**:

- Assist in scheduling and conducting initial assessments with new clients for all ADCS services to determine eligibility and identify client safety and recreation requirements.
- This role will require travel to locations in Burlington, Oakville and Mississauga
- Provide thorough explanation of Acclaim Health services, consent forms, policies and procedures to new caregivers/clients.
- Coordinates and collaborates with other providers to support client service initiation (transportation services, Behaviour Supports Ontario etc).
- Disseminate client information to the staffing teams at the appropriate locations and answer questions that the team may have about client care/safety.
- Create accurate client electronic records in electronic database, upload assessment documents and update service information.
- Provide support to caregivers and respond to inquiries in a timely manner.
- Provide guidance to caregivers on additional support services and complete referrals, as required.
- Participate in team meetings, as required.

**QUALIFICATIONS & EDUCATION**:

- 1-2 years College Office Administration Program or equivalent.
- Valid Ontario driver’s license, use of vehicle and insurance per Acclaim Health policy.

**EXPERIENCE & SKILLS**:

- Minimum of 1 to 2 years of previous customer service/administrative experience.
- Knowledge and understanding of dementia and related disorders.
- Strong verbal and written communication skills.
- Strong interpersonal skills.
- Ability to conduct in-depth interviews with clients/caregivers regarding potentially sensitive health and wellbeing information.
- Effective decision-making, problem solving and organizational skills.
- Demonstrated knowledge in quality and risk management.
- Proven ability to prioritize tasks and meet deadlines.
- Ability to work independently and collaboratively as a team member.
- Previous knowledge of AlayaCare is an asset.
- Valid driver’s license and access to a vehicle.

**Job Types**: Full-time, Fixed term contract

Pay: $26.13-$27.73 per hour

Expected hours: 37.5 per week
