**Job purpose**:
Margaret’s is seeking a hard-working and committed Systems Navigator Coordinator that will report directly to the Director of Programs & Services and will work in the Toronto East Detention Center, agency office, and in the community. The incumbent will be responsible for overseeing program services and supervision of Systems Navigator workers.
- **Responsibilities include, but not limited to, the following**:

- Establish rapport with staff and individuals being released from Toronto East Detention Center
- Coordinate referrals and intake assessments received by the program in accordance with the organization’s policies and procedures such as Client Confidentiality, Anti-Oppression, Anti-Racism, Harm Reduction, etc.
- Oversee direct service to client including information and referral, crisis management, life skills instruction, advocacy, and accompaniment and document according to agency practices
- Maintain friendly and professional conduct and confidentiality when engaging with clients to assess their progress, give support and discuss any difficulties or problems
- Assess clients' strengths and needs, and help with motivation, goal setting, and planning for recovery and reintegration with society and community, considering short-term and long-term goals
- Assist clients with assessing eligibility for social benefits options and develop plans of action while providing necessary advocacy, support, and assistance
- Supervise client activities and assist in pre-release and release planning
- Refer clients to other social services and assist clients in navigating through complex systems, including but not limited to the criminal justice system and locating and utilizing community resources including
- legal, medical and financial assistance,
- housing,
- employment,
- transportation,
- day care and
- other services, as necessary
- Participate in the selection and admission of clients to appropriate programs
- Assist in evaluating the effectiveness of programs by tracking clients' behavioural changes and responses to interventions
- Collaborate with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients' overall progress
- Collaborate with other agencies, organizations, and the City to facilitate community events which provide information, education, and support to individuals
- Recordkeeping and data organization to keep track of program statistics and reporting to fulfill organizational and funder requirements
- Maintain program statistics for purposes of evaluation and research
- Participate in and implement quality improvement and accreditation group and strategies
- Ensure client files are entered correctly and updated often to ensure proper data collection and agency targets are met
- Represent agency at variety of networking events and other meetings as required
- Participate in recruitment, on-boarding and performance evaluation with Director of Programs & Services and Human Resources
- Provide supervision to Systems Navigator workers
- Participate in Coordinator Meetings and represent the program
- Assist with overall coordination of the program
- Engage in fundraising activities and resource acquisitions
- Promote agency program and services, develop partnerships, and liaise with other community agencies and stakeholders as necessary
- Assist with other tasks as assigned and/or required

**Qualifications**:

- Completion of a college or university program in social work required.
- Previous work experience of a minimum of 3-5 years in a social service environment as a volunteer or in a support capacity may replace formal education requirements.
- Knowledge and previous experience of motivational interviewing techniques, conducting client assessments, and use of case management software is preferred.
- Knowledge of issues with regards to mental health, addictions, substance abuse, and homelessness
- Knowledge of designated resources, services, and centers in the community which provide social service assistance
- Capability to work individually and as part of a team to collaborate in providing guidance and assistance to clients
- Ability to maintain client confidentiality and professional boundaries and respect client privacy and autonomy
- Great interpersonal and professional communication skills
- Great organizational and administrative skills
- Willingness and ability to travel within a designated community area
- Ability to speak French and/or another second language an asset

**Working conditions**:

- Working in a designated office space and community area
- Frequent interaction with individuals/families/residents in a community

**Physical requirements  **
- Frequent communication with clients entails going into the community to meet with clients, depending on environmental conditions
- Office work of data entry and reporting requires frequent typing
