**Hours**: 40 Hours/Week (including after-hours and on-call responsibilities)

**Primary Role Purpose**

The Family & Practice Supervisor ensures the provision of high-quality, person-centered family support services that meet the CARF standards of care. This includes oversight of Family Support Workers (FSWs), After-Hours Coordinators, and all family-engaged supports, ensuring planning, implementation, and quality improvement processes align with organizational policies and accreditation requirements.

**Key Responsibilities**

**Personnel Supervision & Competency**
- Supervise, coach, and evaluate FSWs and After-Hours Coordinators.
- Conduct regular performance evaluations based on job competencies and individualized KPIs.
- Ensure staff receive orientation, competency assessments, and ongoing professional development in trauma-informed care, documentation, and family support practices.
- Support staff with timely supervision and constructive feedback as part of continuous skill development and CARF-compliant supervision protocols.

**2. Individualized Family Support & Service Planning**
- Oversee the creation, implementation, and revision of Family Service Plans (FSPs) using a person-centered approach that honors family input and cultural needs.
- Ensure all support plans address strengths, risks, needs, and goals, with measurable outcomes aligned with the individual’s service agreement.

**3. Crisis Response & Safety Oversight**
- Oversee crisis intervention procedures, ensuring timely and competent response to incidents during regular and after-hours.
- Maintain documentation of all incidents and critical events in alignment with CARF and provincial standards, analyzing data for patterns and systemic improvements.

**4. Compliance, Accreditation & Quality Improvement**
- Monitor compliance with internal policies, Alberta Disability Services standards, and CARF accreditation requirements.
- Participate in internal audits and annual quality assurance reviews.
- Collaborate with the Accreditation Consultant to meet continuous improvement standards and maintain evidence of implementation.

**5. Documentation & Records Management**
- Ensure timely, accurate, and confidential documentation of all services, family communications, risk assessments, and service outcomes.
- Maintain and audit staff and client records for completeness, confidentiality, and alignment with policy.

**6. Stakeholder Engagement & Advocacy**
- Liaise with families, guardians, FSCD, Children’s Services, and other community partners to ensure coordinated service delivery and family-centered care.
- Advocate for family needs and rights, helping families navigate service systems and obtain resources.

**Required Qualifications**

**Education**:

- Bachelor’s degree in Social Work, Psychology, Human Services, or a related discipline (Master’s degree preferred).

**Experience**:

- Minimum 3 years in a supervisory or family support role within disability services or healthcare.
- Experience with accreditation processes (CARF or similar) is an asset.

**Skills & Competencies**:

- Proven leadership and team management.
- Expertise in trauma-informed and culturally sensitive practices.
- Strong organizational, communication, and crisis management skills.
- Familiarity with CareSmartz360 and documentation systems.

**Certifications**:

- Valid First Aid/CPR.
- Current Criminal Record Check with Vulnerable Sector and Child Intervention Check.

**Working Conditions & Expectations**
- Full-time hours with flexibility for after-hours supervision and on-call rotations.
- Travel may be required between service sites and for family/home visits.
- Participation in professional development and accreditation readiness
