The **Community Housing Stability Worker** provides proactive, housing-focused case management to individuals experiencing barriers to stable housing. Rooted in the belief that safe, affordable housing is essential to well-being and community strength, this role supports residents of **Tiny Hope**—an inclusive affordable and supportive housing community for adults, youth, and families.

Using a trauma-informed, person-centered approach, the Worker helps residents maintain housing, build essential life skills, and navigate complex systems such as healthcare, housing, and social services. They play a key role in fostering a sense of safety, belonging, and mutual support by offering personalized services tailored to residents’ unique needs.

A central aspect of this position involves designing and leading activities, workshops, and events that promote engagement, skill development, and social inclusion. Additional responsibilities include housing retention support, monitoring of property-related concerns, and collaborating with local partners to deliver comprehensive, wraparound services that enhance long-term stability and well-being.

**Key Responsibilities**

**Client Support & Case Management**
- Use a housing-focused, trauma-informed lens to guide all interactions, interventions, and service planning.
- Manage a caseload of up to 60 individuals, providing housing stability supports and referrals.
- Complete intake documentation and assist with resident onboarding.
- Support the resident selection process, including interviews and eligibility coordination.
- Develop, implement, and monitor individualized support plans, wellness strategies, and crisis/safety plans as needed.
- Conduct home visits, wellness checks, and housing condition assessments.
- Assist residents with navigating systems such as healthcare, mental health, income supports, and legal services.
- Make referrals and facilitate warm hand-offs to internal and external service providers.
- Advocate for resident needs, reduce barriers to services, and promote long-term housing retention.

**Community Engagement & Programming**
- Foster a strong sense of belonging among residents through engagement and inclusive community-building.
- Plan and facilitate life skills workshops (e.g., budgeting, nutrition, communication, resident rights).
- Coordinate and run weekly programs for priority populations (e.g., women-led families, youth, seniors).
- Support and encourage participation in the community gardening initiative.
- Address food insecurity by connecting residents with resources and food supports.
- Engage with local organizations to bring in additional programming based on resident interests and needs.

**Property & Community Safety**
- Perform daily property checks and collaborate with property maintenance on resident and unit-related concerns.
- Support overall community wellness and safety through daily engagement and monitoring.
- Attend inspections, case conferences, and team/community meetings as required.

**Collaboration & Administration**
- Coordinate with community partners to ensure wraparound care and targeted programming.
- Maintain accurate records, including detailed case notes and program statistics.

**Qualifications & Experience**
- Bachelor’s degree in Social Work (BSW) or a related human services field.
- Minimum 3 years of experience in housing support, social services, or community-based case management.
- Strong knowledge of trauma-informed care, harm reduction, and housing-first principles.
- Experience with resident intake, support planning, group programming, and crisis response.
- Familiarity with the Residential Tenancies Act, Mental Health Act, and local support systems.
- Excellent communication, relationship-building, and de-escalation skills.
- Strong documentation skills and proficiency in Microsoft Office (Word, Excel, Outlook).
- Valid driver’s license, reliable vehicle, and adequate insurance coverage.
- Ability to work independently with little supervision.
- Willingness and availability to work evening hours to accommodate after-hours programming.
- Clear Police Vulnerable Sector Check required prior to employment.
