**Choices is offering an exciting 11 month contract** for a Health Services Coordinator. This is an ideal contract for a new nursing graduate or someone looking for a new challenge who is interested in health promotion, developmental disabilities, disability advocacy and social services. This position is offered as part of a Resilient Communities grant, which assist organizations in rebuilding after COVID-19.

**Bona Fide Requirements**:
Engage in all aspects of behaviour management; be licensed to drive in the province of Ontario; administer medications as per policy; obtain a Vulnerable Sector Screening Check and obtain a valid first aid certificate and other mandatory education as deemed appropriate by Choices.

**Objective/Purpose of the Health Services Coordinator Position**:
The Health Services Coordinator is a Registered Practical Nurse (RPN) and is responsible for supporting individuals in reaching their full potential as a member of our community and in all physical and mental health related matters. The Health Services Coordinator advocates for individuals so they may live in a state of dignity, share in all elements of their community and have the opportunity to participate effectively.

The Health Services Coordinator is responsible for providing health supports and interventions across all programs at our Choices SGLRs, Community Participation locations and to Supported Individuals living in the community and within Host Families.

The Health Services Coordinator is also responsible for communicating the health needs of individuals to our front line direct support workers and to support staff in making appropriate decisions when providing care.

This position will have opportunities to participate on committees that oversee health and safety, IPAC and pandemic planning. The Health Services Coordinator also supports the Workplace

Wellness & Safety Manager by teaching and monitoring unregulated care providers who are performing exempted controlled acts and advanced health care skills.

**Principal Roles/Responsibilities**:
The job responsibilities include developing, monitoring, and delivering professional nursing care and health promotion to people supported at Choices, including, but not limited to:
**_Coordination and Implementation of Health Services: _**
- Ensure the daily health care needs of people supported by assessing, planning, evaluating, and recommending an optimal health care plan, where interventions can be managed autonomously within the nursing scope of practice and according to Physician’s orders.
- Create and update Plans of Care
- Provide health updates on people supported to the staff team and families.
- Provide mentoring, monitoring, and training to staff by working as part of the team to; enhance the knowledge of staff in early detection of illness; provide health related information to staff that is evidence based; identify training needs of staff to the supervisor; provide training that is within the nursing scope of practice and monitor the effectiveness of training; encouraging a supportive and cooperative team to support individuals with both their physical and mental health.
- Acts as a liaison between people supported, health care teams and families where needed.
- Assist the Workplace Wellness & Safety Manager with workplace wellness programs and education including health promotion, public health education, medication administration, first aid, abuse education and infection prevention and control protocols.
- Attend annual training as required and any additional training as needed.
- Respect confidentiality, following appropriate lines of communication.
- Perform RPN duties in a consistent manner with agency policies and procedures.
- Work professionally and in cooperation with colleagues participating effectively as a team member, and in compliance and in good standing as a member or the College of

Nurses of Ontario.

Networking: Will collaborate and network with health care teams, public health and other agencies as needed to enhance the lives of the people supported. Participate in staff meetings with the Program Manager and follow up with additional supports as needed.

Responsible to support the Program Managers on all required health documentation. Works with the Quality Assurance Manager to ensure all sites maintain full compliance at all times as per Choices policies and MCCSS directives.
