**INTAKE FACILITATOR**

**Clinical Administrative Support Services (CLASS)**

**Permanent Full Time**

**37.5 hours/week**

**Brampton Site**

**Work Arrangement: Remote**

(2 days on-site/3 days remote)

**Key Responsibilities**:

- Obtain eligibility information from clients and assist them to access services of the appropriate program and provide proper referral information to community agencies in the event of ineligibility.
- Receive, process and follow up on all incoming referrals from hospitals, physicians, community and other Regional IH Coordinating Agency sources.
- Maintain and complete up-to-date client records.
- Respond effectively to all telephone enquiries from prospective clients, families and agencies regarding ErinoakKids services.
- Assess the urgency of client needs and priorities as per program triage criteria, while managing client expectations.
- Provide support to the CLASS team in working a 10-6 shift as required.
- Attend staff meetings and training updates as required.

**Qualifications**:

- College Diploma in a related field (Health/ Administration/ Human Services).
- Minimum 1 year of similar work experience.
- Experience in the CLASS department preferred.
- Superb interpersonal and communication skills (written and verbal) to deal effectively with staff, clients, volunteers and others within the community.
- Excellent computer skills (Microsoft Office: Intermediate Word and Excel).
- Exceptional organizational and problem solving skills.
- Strong attention to detail.
- Ability to work independently with mínimal supervision.
