- Coordinated Service Planning supports children and youth with multiple and/or complex special needs by providing their families with a seamless, family-centered service experience. The goal is to reduce family stress by helping them navigate and coordinate services, while ensuring their voices are central to the planning process.

Grounded in the principles of family-centered care, inclusive collaboration, and seamless information sharing, Coordinated Service Planners work with families and service providers to develop a strengths-based Coordinated Service Plan. This plan reflects the child or youth’s goals and needs and promotes participation in home, school, and community life.

**GENERAL RESPONSIBILITIES**:
**Coordinated Service Planners will**:

- Actively engage children or youth and their families in Coordinated Service Planning, including goal setting, using a strengths-based, solution-focused, and family-centered approach
- Facilitate the exchange of information among service providers across the children’s services, education, and health sectors to develop and maintain a Single Plan of Care (SPOC)
- Connect families to appropriate services, supports, and community resources within the service delivery area
- Identify and advocate for flexible and innovative approaches to service delivery; escalate any barriers to innovation that may arise
- Continuously monitor, review, and update the Single Plan of Care (SPOC) in collaboration with the child or youth, their family, and relevant providers, as needs and services change

**DUTIES PERFORMED BY A COORDINATED SERVICE PLANNER**
- Facilitate case conferencing with multiple agencies, with consent and the active participation of the child or youth and their family
- Develop a strengths-based, goal-driven Single Plan of Care (SPOC) that addresses service needs and supports participation at home, in school, and in the community
- Lead the full process of Transitional Planning in accordance with the framework of the Integrated Transitional Protocol
- Provide information to the Partnership Circle as requested and attend meetings as requested by the Chair and Supervisor
- Build and maintain effective working relationships with families and partner agencies using a person-directed approach
- Maintain clear, accurate, and confidential records (e.g., case notes, statistics, SPOCs) and produce written/verbal reports as required, in compliance with legislation and agency policies
- Adhere to all FCSRC policies and procedures, Ministry Regulations, Standards, Directives and relevant legislation.

**PERFORMANCE INDICATORS**

Demonstrated use of a strengths-based, solution-focused approach and associated tools in case planning and documentation
- Clear trajectory of service within case files, including intake, needs assessment, support provision, SPOC development, advocacy, and transitional planning
- Families consistently report having a clear and reliable point of contact in their Coordinated Service Planner and understand who is accountable for their child of youth’s Coordinated Service Plan.

**SKILLS/ABILITY REQUIREMENTS**:
Demonstrated ability to work effectively with a diverse service and staff population, and a proven commitment to anti-oppression, equity, and inclusivity in practice.
- Strong critical thinking, advocacy, and evidence-based decision-making skills
- Excellent written and verbal communication skills
- High level of professional self-motivation, time management, and organizational skills
- Ability to collaborate toward shared outcomes and to support the empowerment and independence of families
- Ability to maintain professional relationships and build both personal and professional support networks

**KNOWLEDGE &**EXPERIENCE**
- A two-year community college diploma in Social Sciences, Community Services, or a related field; or an equivalent combination of education and related work experience. Minimum one year of experience in a related field, ideally in community-based case management (Health or Developmental Services preferred)
