**Adult Day Program Worker**

**15-month - Full time contract (35hrs/week)**

**Salary Range: Starting at $50,785.00**

**Description of Responsibilities**

Reporting to the Community Support Services Program Manager, the Adult Day Program Worker will be a member of the Community Support Team and will be accountable to the Board of Directors. The Adult Day program is an integrated support service which provides supervised programming in a group setting to clients such as those who are frail and elderly and living with Alzheimer’s or related dementias or living with a physical impairment. This service assists the clients to achieve and maintain their maximum level of functioning, to prevent early or inappropriate institutionalization and provides respite and information to their caregivers.

**Responsibilities**
- Co-design and set up specific program activities using programming that will positively stimulate the client group, both cognitively and physically
- Deliver the prepared programming and work directly with clients during the program hours
- Conduct assessments of new clients, set care plans and facilitate integration to the program
- Provide support to care partners
- Organize the daily schedule of activities
- Complete intake process for all inquiries for Community Support Services, record information from referral source and provide follow-up.
- Maintain a neat, safe and pleasant environment for the program’s efficient and effective operation
- Work with other staff and volunteers to provide effective services
- Assist in the orientation of volunteers and students to the program
- Maintain accurate records on clients and volunteers
- Perform administrative functions

**Qualifications**
- Community College Diploma or certificate in nursing, social work, gerontology or a related field
- Knowledge of Alzheimer Disease and other related disorders
- Excellent communication skills and strong interpersonal skills
- Ability to work and function as a member of the Community Support team and

the larger resource Centre team
- Ability to support volunteers
- A willingness to maintain high quality services, client centered services through superior organizational and problem-solving skills
- Computer competencies (Word, Excel, Outlook, CIMS)
- Oral and written fluency in English and French
- Ability to work evenings and weekends
- Valid Driver’s License and willingness to drive clients with EORC vehicle
