Practitioner Part-time, 11pm-7am(Sleep) and 7am

**This position is every second weekend Friday and Saturday Sleep Night (11pm-7am) with a 7am - 3pm, (32 hours total).**

The Resident Support Worker position provides high-quality support to individuals based on their needs and wishes as defined in their person-centered plans, support plans, risk assessments, and in keeping with the philosophy and aims of Cosmos Group of Companies.

**Key Responsibilities**
- Create and maintain a positive professional relationship with residents to ensure that they are fully involved in all decision-making processes that affect their lives.
- Monitors clients’ well-being. Assists clients with daily life skills and/or behaviour management training. Assists clients with basic cooking, housework, yard maintenance, etc.
- Assists clients in attending appointments when necessary and may provide transportation.
- Provides physical care as required, which could include bathing, toileting, dressing and feeding as necessary.
- Support resident’s in all aspects of the personal care ensuring the maintenance of their dignity and privacy at all times
- Administer necessary medication to residents following appropriate training and assessment and to support individuals to self-medicate where it has been assessed safe to do so.
- Encourage and facilitates the participation of individuals in the program. Create a fun and enjoyable atmosphere in order to enhance the program’s appeal to clients.
- Recognize potential emergency situations, analyze situations accurately, and develop strategies to deal with such situations i.e. dispensing medication, and dealing with client’s aggressive behaviour to ensure no harm comes to the client, staff and/or the public.
- Observe residents to ensure that their physical and mental wellbeing is intact and report any significant changes
- Review and evaluate clients’ progress and makes adjustments to programs as required. Provides feedback and support to fellow staff and clients.

**Additional Expectations**:

- Keeps clear, concise, and accurate records of all aspects of care provided, resident’s needs, issues within the home, Accidents/Incidents, medication records, daily records, health and safety monitoring, and all other documentation required as part of monitoring practices.
- Ensures that records are written professionally and are stored in accordance to Cosmos Group of Companies policies and procedures, and FOIP.
- Disclose any knowledge, suspicion, or concern relating to the alteration or falsification of any documentation which you may discover, witness, or learn of.
- Promotes effective communication within the team, and with family, friends, community members and other agencies.
- Ensures that relevant information is passed on and recorded when necessary and is communicated to residents in a manner that is appropriate to them.
- Demonstrates confidentiality in all matters relating to residents, the home, and Cosmos Group of Companies; not disclosing information unless authorized or appropriate to do so.
- Maintains a professional attitude at all times when discussing service-users, the home, and Cosmos Group of Companies.
