**Position Summary**:
**Qualifications**:
- 3 years experience working with seniors -** Required**
- Ability to work independently and within an interdisciplinary team
- Experience conducting Cognitive assessments/screening tools related to the geriatric population (ie. CAM, MOCA, GDS, Cornell, GAS, etc.)
- Good knowledge of mental health/geriatric support services in Windsor and Essex County
- Excellent interpersonal and communication skills with all clients, staff, visitors of HDGH as well as our partnership organizations
- Clinical skills to assess signs and symptoms of psychiatric illness
- Experience with psychosocial and risk assessments
- Good problem solving ability, and work well under pressure
- Ability to work independently and in partnership with community partners
- Proven experience in responsive/expressive behaviours
- Valid Ontario Driver’s License, means of transportation and ability to travel on a daily basis for outreach services**:

- There will be an interview conducted with both HDGH and the requisite community partner program (Behaviour Supports Ontario-BSO).
**Education and/or Certification**:
Bachelor or Master of Social Work, Psychology, or Social Sciences - **Required**
- Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers**:
- Required**
- Previous Specialty Geriatric Education (I.e. PIECES/GPA, etc.)
- **Preferred**
