The Toronto Grace Health Centre (TGHC) provides specialized care and services to individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, Wound Care, Respiratory Therapy, and Transitional & Remote Care. The care and services provided at TGHC helps facilitate patient flow through the health care system and ensures recovery and supports their integration back into the community.

Reporting to the Director of Clinical and Transitional Care Programs, the Manager of Patient Flow is responsible for the optimization of beds across the Toronto Grace Healthcare Network.

The Manager of Patient Flow works collaboratively with external stakeholders across the Healthcare system and internal Management Teams. The Manager provides day to day supervision to the Admitting and Flow Coordinator. This role is an exciting opportunity for a highly motivated individual who excels in working with an interdisciplinary team and is able to develop, implement and evaluate initiatives that advance the strategic directions of the hospital.

**KEY RESPONSIBILTIES**
- Develop relationships with external stakeholders to create a conduit for current and future patient admission into the Toronto Grace Health system.
- Coordinate patient flow from internal and external stakeholders to ensure all patients are admitted and referred to the most appropriate services for their clinical needs
- Create reports to inform management and clinical leaders about bed utilization trends
- Provide supervision and support to the Patient Flow and Utilization Coordinator

**EDUCATION AND QUALIFICATIONS**
- A Bachelor’s Degree in a related field required
- Must be a regulated healthcare professional
- All regulated Health Professionals must be in good standing with their professional organization or college.
- A minimum of 7 years of clinical experience in acute care and rehabilitation settings
- Good knowledge of community based and long term care programs and services and applicable legislation
- Experience managing competing demands in a fast-paced, multi-site, multi-disciplinary health care environment
- Self-directed; ability to handle a wide variety of administrative tasks
- Well-organized with an ability to multi-task, prioritize and meet deadlines
- Superior interpersonal skills and high degree of emotional intelligence
- High degree of professionalism, integrity, maturity, discretion, initiative and judgment
- Excellent oral and written communication skills
- Demonstrated proficiency in Microsoft Office; advanced knowledge of Word, PowerPoint, Outlook, Excel)
- Excellent attendance record. Must have physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Occupational Health Services
