Ambulatory Services’ Adult Day Program (The Samuel Lunenfeld Moutainview Club)has an opportunity for a

**PROGRAM ASSISTANT, PERMANENT FULL TIME**

SEIU Service Hourly Pay Rate: $23.5504 - $24.1549

**The Adult day Program **is focused on offering older adults a wide range of social, cognitive, physical and spiritual activities. This program is designed for community dwelling seniors with physical frailty as well as moderate to severe cognitive impairments.

**Responsibilities include but are not limited to**:

- Providing safe and effective direct care and support during programs in order to meet the needs of program members as identified in the interdisciplinary care plan
- Supporting the independence, dignity, social, emotional and cultural well-being of program members
- Promoting an environment that is conducive to dementia care, mental health care and physical frailty in a community setting
- Supporting the delivery of in-person and virtual programs related to health and wellness of Program Members
- Planning and conducting assigned programs as well as initiating new programs and planning of special events under the guidance of the Therapeutic Recreationist team
- Supporting members being transported to and from the program while on the Baycrest bus
- Initiating and maintaining relationships with the client, their family and other care providers
- Some evenings and weekends may be required.

**Qualifications include but are not limited to**:

- Current P.S.W. certificate from a recognized community college or educational institute or equivalent combination of education and experience
- Experience in program activation/ recreation preferred
- Activation Diploma preferred
- Current Basic Cardiac Life Support (BCLS)
- Certificate in Dementia Care
- Ability to work independently and as a part of the interdisciplinary team, and effectively managing challenging situations including the behavioural and psychological symptoms of dementia
- Excellent interpersonal, communication, organizational and problem solving skills
- Experience working with at-risk or frail seniors living in the community preferred
- Experience delivering services and care to seniors with cognitive impairment, mental health issues and complex medical issues
- Experience working in a Day Centre or community service setting preferred
- Knowledge of Yiddish, Hebrew, Russian languages an asset
