**Department: Behavioural Supports Ontario MOT**

**Site: Hogarth Riverview Manor**

**Shifts: Days/Evenings including weekends**

**Rate of Pay: $21.46 - $22.83 per hour**

As a member of the Behavioural Supports Ontario Mobile Outreach Team, the Personal Support Worker will provide strategies and support to family and staff caring for individuals with various types of responsive behaviours who are residing in a Long Term Care Home within the City of Thunder Bay. The outreach staff will work as part of an interprofessional team under the direction of the Manager, Specialized Geriatric Services.

**Qualifications**:

- Current Personal Support Worker certificate from a recognized college program
- Demonstrated knowledge and skill working with individuals with various types of dementia, delirium, and mental health issues
- Certification relevant to practice setting (ie Gerontology, Dementia), including training in Gentle Persuasive Approaches (GPA) and U-First is an asset
- Current Basic Cardiac Life Support (BCLS) is an asset
- Demonstrated familiarity with the Long Term Care (LTC) Home Act and Regulations is an asset
- Excellent interpersonal and communication skills as well as critical thinking and problem-solving abilities relative to the situation and scope of practice
- Mentorship and coaching skills
- Demonstrated commitment to client centered care
- Ability to work independently and as part of a team, with mínimal supervision
- Will be required to travel to various work locations and provide own transportation
- Demonstrated regular attendance at work

**CONDITION OF EMPLOYMENT**
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
