Position Summary
Reporting to the Health & Recovery Services leadership team, the Care Facilitator provides consistent weekend coverage to ensure operational continuity and team stability across both the Urban Response and Recovery Oriented System of Care (ROSC) programs. This role supports frontline staff in maintaining high standards of care and service delivery, with a focus on facilitating communication, resolving challenges in real time, bridging programmatic gaps in the absence of weekday leadership, and ensures ongoing collaboration with the Emergency Shelter programs. They will also provide hands-on support across both programming areas, which may include direct client care, coordination of services, administrative duties, and assisting with staff development.
With a deep understanding of the roles and workflows within both departments, the Clinical & Program Facilitator offers hands-on support, participates in crisis management, manages shift-level logistics such as staffing, inventory, and bed flow, and helps ensure safe, coordinated care for clients. This role does not introduce a new layer of formal leadership but rather serves as a stabilizing and knowledgeable presence during extended hours.
Time Commitment
40 hours per rotation with 4 days on and 3 days off. Some unusual hours may be required from time to time as the need arises. Schedule may be altered at the discretion of management.

**Responsibilities**:

- Maintain operational stability across both Urban Response and ROSC programs during weekend shifts.
- Manage unexpected staffing gaps (e.g., sick calls) by finding coverage or stepping in as needed to maintain service levels.
- Attend and contribute to weekend crew meetings to ensure cross-department alignment and communication.
- Support staff from all disciplines, including LPNs, Recovery Care Workers, Peers Support Workers, and Mental Health & Addiction Navigators.
- Make informed decisions in complex or high-stakes situations, with staff feedback and input, such as in the case of behavioural issues and determining whether to discharge from programming or other unique situations as they arise.
- Act as an escalating point for client complaints or urgent issues; intervene in crises and support de-escalation efforts when needed.
- Ensure staff attend mandatory training sessions and are able to take required breaks, providing coverage or arranging coverage as necessary.
- Provide orientation and light training support for new staff when requested.
- Perform light administrative tasks to support program documentation and flow, including incident report completion and maintaining EMR data integrity.
- Assist with bed management and client flow through ROSC programs (intake/discharge coordination).
- Monitor inventory and supplies to ensure essential materials are available and properly stored.
- Sterilize and reprocess medical equipment, as required.
- Foster a collaborative and trauma-informed environment that supports both staff and clients.

Required Competencies and Qualifications
- Completion of an accredited healthcare provider education program (two-year Practical Nursing diploma).
- Registration with the College of Licensed Practical Nurses of Alberta (CLPNA). Proof of registration, in good standing with no restrictions or conditions, required.
- Valid Professional Liability Insurance. Proof of coverage required.
- Satisfactory Criminal Record and Vulnerable Sector Check - dated within the last 6 months. Copies required.
- 2-4 years of nursing experience required, preferably within the addictions and mental health field and/or the homeless serving sy
