Under the overall supervision and reporting to the Director, Health and Community Services, the Social System Navigator (SSN) will be a member of our interprofessional team, steering our clients through a complex social and health system to access resources and services. The SSN is an expert in working with the social determinants of health and will be dealing with issues related to health care access and supports, housing, finances and community and government structures.

The SSN will:

- With individuals and families, use multi-faceted psychosocial assessments of needs that consider strengths and the social determinants of health in primary care, chronic disease prevention and home environment.
- The SSN will provide counselling, education, advocacy, and coordination as they work with primary care providers, mental health professionals and the chronic disease management team as well as other professionals and service providers to ensure the clients’ care plans are carried out or modified effectively.
- The SSN will work to identify issues and gaps in services and will advocate for those issues and gaps to be addressed and filled.
- Through psychosocial support, active listening and crisis intervention, the SSN will explore possible approaches to address issues when individuals and families are dealing with new, chronic and more acute health care needs.
- The SSN will develop external relationships with other health agencies, government, institutional and societal systems to advocate on behalf of clients for necessary services, and against systemic discrimination.

**REQUIREMENTS**
- Completion of Bachelor of Social Work or Master’s Degree in Social Work
- 2-3 years relevant work experience in a community based and / or primary health care setting
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or be eligible for registration
- Knowledge of the urban Indigenous community and experience working with Indigenous persons
- Knowledge and experience in identifying community-based programs, services and resources
- Ability to work with a high degree of independence while collaborating with an interdisciplinary team
- Ability to problem solve, exercise independent judgement, assess situations and address problems efficiently and effectively
- Excellent interpersonal, communication and organizational skills
- Proficiency in computer and good keyboarding skills
- Awareness and respect of Ojibway /Cree traditional healing practices
- Ability to speak Ojibway/Cree is an asset
- Submit to a Tuberculin (TB) Skin Test, Satisfactory Criminal Records Check including Vulnerable Sector Screening Check upon job offer

Pay: $41.69-$49.88 per hour
