For more information on WPSHC, please visit our website. **About the Opportunity**: We are currently looking for a **Social Worker** to join our inter-disciplinary team of skilled professionals. As a member of the multi-disciplinary team in the Department of Patient and Family Centred Care, the Social Worker in this role will support Discharge Planning and the Community Stroke Rehabilitation program. The Social Worker will provide consultation to patients, families, Physicians, and others to facilitate individualized, safe, and timely transitions home or to the next level of care, ensuring quality and continuity of patient care. They will also provide Social Work assessments and interventions to patients and families involved in the outpatient Community Stroke Program. You will also need to have: - Current certificate of registration in good standing from the ON College of Social Workers and Social Service Workers - Minimum three (3) years of experience working with patients who have had a stroke, other neurologic, complex chronic conditions and/or musculoskeletal disorders in a rehab or community setting is preferred - Certification in Cognitive Behavioural Therapy, Motivational Interviewing, and Mindfulness-based stress reduction, preferred - Completion of CorHealth’s Interprofessional Stroke Core Competency Framework, or willingness to complete. - Demonstrated comprehensive knowledge of community services, agencies, and placement facilities. - Demonstrate knowledge and understanding of legislation and acts applicable to patient discharge and transition (e.g. Health Care Consent Act, Mental Health Act, Public Hospital Act, Substitute Decisions Act) - Demonstrated knowledge of financial aid available from federal, provincial, and municipal programs. - Excellent communication skills and collaboration when interacting with patients, family members, and other members of the care team. **_Please note: _**_For the safety and well-being of our clients and employees, we require proof of COVID-19 vaccination before commencing employment.
