**Job description**

PREFERRED QUALIFICATIONS AND EXPERIENCE:

- Minimum of a diploma or undergraduate degree in the Human Services field. B.S.W. would be considered an asset or 2 years of working directly with children and families.
- Educational equivalency and experience will be considered as per agency policy.
- Strong assessment, communication, organization, and writing skills.
- Ability to develop, monitor, and evaluate appropriate support plans.
- Ability to work effectively within a multi-disciplinary team.
- Strong group facilitation and training skills.
- Knowledge, understanding, and acceptance of Indigenous and diverse cultures.
- Competence in using Microsoft Office.
- A valid Class 5 Driver’s License and reliable vehicle.
- Valid First Aid
- CPR/AED
- A current, clear Police Information Check (vulnerable sector included) and Child Intervention Record Check, both dated within the last 6 month
- Knowledge of Trauma-informed practices, parenting strategies, and behaviour management strategies.
- Good understanding of health issues affecting youth and adults, such as FASD, drug and alcohol addictions, intergenerational trauma, mental health, suicide and other mental and behavioural issues.

GENERAL RESPONSIBILITIES:

- Ensure the care and behaviour management policies and procedures are met within the foster homes.
- Coordinate and participate in case conferences as deemed necessary.
- Coordinate and participate in placement orientated service team meetings as needed.
- Conduct crisis intervention regarding children and youth in the Agency foster care program and support team members involved in crisis situations, as required.
- Request and coordinate academic, psychological and psychiatric assessments as required.
- Conduct face-to-face contact with child/youth and caregivers according to Agency and government standards.
- Facilitate intake into and discharge from the Agency’s foster care program.
- Provide caregivers with relevant child-related information at the time of placement and as subsequent information become available.
- Assist caregivers in developing support systems and resources within their community.
- Ensure clients are given the opportunity to be actively involved in culturally appropriate activities.
- Ensure outcome evaluations and service plans are applied to individual client cases in accordance with Agency policy.
- Maintain and update child based outcomes, as indicated by the Agency.
- Ensure the case management process is logically, concisely and legibly recorded and reflects a good standard of grammar and content reflects the case management process.
- Ensure that the meeting is organized and conducted in a professional and effective manner.
- Ensure that report and meeting deadlines are met.
- Foster Care Support Worker must submit all support/service plans, outcomes, and contact notes to the Program Supervisor for approval.
- Ongoing supervision and annual evaluation of caregiver skills and abilities.
- Ensure Agency reporting and evaluation mechanisms are completed on a timely basis, as required.
- Ensure caregivers are meeting Agency, legislative and accreditation standards.
- Maintain all requirements as per the Child Management Program (C.M.P.).
- Responsible for providing 24-hour on-call services and covering back-up, as scheduled.
- Ensure placement of children is appropriate, based on the child’s needs and the skill level of the caregivers, by Agency policy and standards.
- Ensure all homes meet licensing and training requirements.
- Update the Program Supervisor of home vacancies and any changes in the homes.
- Complete home studies and addendums as scheduled by the Executive Director and the Program Supervisor.
- Assist in the development and delivery of Agency caregiver training.
- Facilitate caregiver support groups and child/youth groups.
- Other related duties as requested by the Executive Director and Program Supervisor.
