We are currently looking to hire a:
**Palliative Care Coordinator**
**1 Full Time (35 hours/week)**

**Reporting To**:Manager, Community Health Teams

**Purpose**
The Palliative Care Coordinator is responsible for the intake, assessment and service provision for clients facing a life threatening illness. They will be involved in case management and case consultations with team members and other health professionals in Scarborough. The Palliative Care Coordinator will also oversee the efficient and effective operation the Day Hospice program and Caregiver Support Group.

This position demands strong leadership skills, flexibility in hours, the ability to handle stress, and the ability to understand and recognize compassion fatigue in volunteers, along with the need for self-care.

**Scope and Accountability**
- Client intake & Assessment_
- Collect and enter referrals into the health record
- Complete home visits to conduct the assessment and complete the required forms to initiative services
- Develop, implement, and regularly evaluate a plan of care
- Make home visits as needed
- Make referrals to other services as needed.
- Volunteer Matching & Management_
- In collaboration with the Coordinator of Volunteers, assess suitability of potential volunteers for front-line services
- Identify suitable client-volunteer matches
- Introduce the client to the volunteer as needed
- Maintain regular contact with all clients and matched volunteers
- Document in the health record
- Collect, review and enter volunteers statistics into the database monthly
- Keep abreast of issues in volunteers’ lives (e.g. absences, sickness, bereavement leaves)
- Participate in appreciation and recognition events annually
- Participate in support/educational activities for volunteers as needed
- Respond to and resolve client and volunteer complaints
- Office Administration_
- Document according to organizational policy and maintain volunteer information
- Review current forms and develop new forms when appropriate
- Develop, review, and distribute all client policies and procedures to volunteers and staff
- Community Collaboration_
- Attend meetings and events in the community that enhance Scarborough Hospice’s profile, ability to strengthen services and partnerships
- Build relationships with other individuals and agencies to provide strong referrals and case collaboration for more challenging situations
- Establish and maintain key relationships with doctors, pharmacists, staff at long-term care centers, social workers, and others for the purpose of effective referrals and program development.
- Risk Management_
- Monitor risk management policies and procedures to ensure that programs and organization risks are minimized
- Evaluate the risks associated with each client and take appropriate action to control the risks
- Ensure the volunteers are able to work in safe, healthy and supportive environment in accordance with all appropriate legislation and regulations
- Ensure the clients have current information regarding their rights and our limitations
- Other duties as required.

**Educational and/or Professional Qualifications**
- University degree or college diploma in nursing, health services, volunteer management, social work, or other related field.
- Fundamentals of Palliative Care or related certificate.

**Level of Experience**
- 3-5 years relevant experience in the nonprofit sector.
- Strong understanding and experience with client centered practice, group facilitation and case management.

**Skills and Attributes**
- Experience with individual assessments and case management
- Knowledge of current trends, resources, and information related to hospice.
- Knowledge and experience working with life threatening illnesses in a hospice/palliative care setting
- Excellent problem-solving, decision-making, and leadership skills
- Strong communication, verbal, listening and writing skills
- Strong assessment, problem solving, and leadership skills
- Sensitivity to and awareness of cultural, racial and socio-economic diversity within the community.

**Working Conditions**
- Flexible work schedule - occasional extended hours and evening work will be required.
- Physically, the office is located in an accessible building however; assessments are done in the clients’ homes.
- Valid driving license.
- Vehicle required.

**Remuneration**: $27.68 - $32.32
