Job Description

The Day Centre, Program Assistant reports to the Registered Nurse and provides operational and personal assistance to the clients. The Day Centre, Program Assistant is also responsible for a variety of administrative and coordination services in support of the Day Centre program.

**We are looking for**: 1 full-time, permanent Program Assistant

**Salary**: Commensurate with experience

**Hours of Work**: 34 hours per week. Role is based at the Joseph & Wolf Lebovic Jewish Community Campus 9600 Bathurst Street, Maple

**Reports to**: Supervisor, Adult Day Program

**Responsibilities**:
Provides personal care to designated clients according to established care plans
- Assists clients with personal care including bathing, dressing, grooming, personal hygiene, mobility, feeding and other routine activities of daily living.
- Encourages and assists clients to perform activities of daily living and community living skills within their capabilities.
- Identifies, observes and reports to the RN or Manager any circumstances which affect clients and service.
- Recognizes and respects client's rights which include informed and quality services and confidentiality.

Customer Service and Administrative Support
- Effectively meets all client services requirements in a courteous and professional manner
- Proactively trouble shoots problems, deals with difficult situations, and escalates to RN or Manager when extra assistance is required.
- Participate in client satisfaction initiatives that continuously improve quality levels.
- Communicates effectively with clients, family members and the care-giving team.
- Reports to the RN concerns about changes in the client's condition and/or care plan.

Team Building and Leadership
- Actively participate in team initiatives and meetings
- Build support for agency vision and departmental goals/objectives by communicating, collaborating and leading by example
- Build rapport with team members to ensure a high performing team
- Look for daily opportunities to highlight the connection between overall strategy and day-today activities
- Act as a change agent to positively encourage others and manage change

Health and Safety Management
- Responsible for identifying health and safety concerns and injury reporting
- Participate in health and safety processes and procedures
- Participate in maintaining a safe workplace environment
- Participate in all health and safety training initiatives

**Qualifications**:

- PSW Certificate, Certified Health Care Aide or Activation Diploma
- Basic Cardiac Life Support Certified
- Public Health Food Handlers Course (an asset)
- Experience working with frail and impaired elderly and their families
- Experience working with individuals living with Alzheimer’s disease and dementia
- Experience working with clients in a Jewish environment
- Experience in recreational programming for seniors
- Basic computer and writing skills
- Clear communication skills
- Experience working as part of multi-disciplinary team
- Language skills are an asset (e.g. Yiddish, Russian)
