The Social Worker counsels and provides guidance to individuals and/or families with various social, medical and/or psychiatric challenges, under the guidance of the Director of Care.

**Responsibilities**
- Helps resident and family members through individual or group conferences to understand, accept, and follow medical recommendations
- Supports residents and families with problems concerning relationships or other aspects of social functioning affected by disability
- Collaborating with the multidisciplinary teams in diagnosis and treatment plans
- Conducts psychosocial screening activities determining required specialized psychiatric assistance
- Serves on committees as required
- Performs other related duties as assigned

**Qualifications and Experience**
- Must be a Registered Social Worker or eligible for registering
- Bachelor’s degree in social work, or one of the following Counseling, Psychology, Behaviour science or related field
- Minimum 2 years experience working in a social services field or related capacity; or any equivalent combination of experience and/or education from which comparable knowledge, skills and abilities have been achieved.
- Preferred experience in a Long Term care setting.
- Knowledge of the principles, practices, techniques, and professional standards in the field of social work
- Understanding of community organizations and social service programs available
- Strong counselling and conflict resolution skills
- Exceptional interpersonal skills with peers, residents, visitors, and operational partners
- Solid organizational skills, demonstrate the ability to prioritize and problem solve
