The Ubuntu CSD (Collaborative Service Delivery) Program is an innovative approach to child and family support, which leverages a network of partner agencies and Children’s Services to create customized support for children, youth, and families in Northeast Edmonton.

Ubuntu offers a continuum of culturally aligned, community-based, family and child centered services. Collectively, with Boyle Street as the lead agency, Ubuntu is one point of access to a streamlined set of services that leverages the expertise of five networks to meet the needs of our families, children, and youth. Entitled the C5 (Collaborative Five) - Ubuntu works collaboratively with Boyle Street Community Services, Bent Arrow Traditional Healing Society, Edmonton Mennonite Centre for Newcomers, Norwood Child and Family Resource Centre, and Terra Centre for Teen Parents.

**JOB PURPOSE**:

- The Ubuntu CSD (Collaborative Service Delivery) Program is seeking a Family Support Worker; an integral role in our team, responsible for working with families to coordinate services and build relationships to achieve family goals. The Family Support Worker works directly with children and families in Northeast Edmonton in a shared practice model with partner agencies and Children’s Services.

**KEY AREAS OF RESPONSIBILITY**:
- Work in collaboration with family members, Children’s Services’ Assessors and Caseworkers, as well as partnering community agencies to develop a Children’s Services Planning Form (CSPF) for the family-
- Collaboratively design the CSPF to include tasks assigned to specific individuals to work towards specific goals that relate to the safety of children, with an intentional consideration on Ubuntu’s four pillars (children, family, community, and culture)-
- Focus time spent with family (whether a formal meeting, driving to appointments, and/or phone and text coordination and check-ins) with the intention of working towards reaching the service plan goals-
- Coordinate and attend monthly service team meetings with all the service providers and natural supports involved-
- Locate and connect family members to community resources, especially within the C5 partnership and Northeast Edmonton-
- Role modelling healthy boundaries and relationships with family members and collaborative community partners-
- Build positive relationships with children, families, the Northeast Child and Family Services Neighborhood Office staff, and multi-disciplinary partners to better support families-
- Contribute to a positive team environment by demonstrating the values of accountability, respect, and creativity-
- Advocate on behalf of families, and teach self-advocacy-
- Responsible for on-call services on a rotational basis-
- Maintain accurate records, files, and documentation to track outcomes**QUALIFICATIONS**:

- Minimum 3 years’ working with families and experience conducting home visits and case management-
- Understanding of Child and Family Services and guiding legislation-
- Knowledge of and experience with Indigenous and multicultural communities is essential, particularly how they have been affected by the child welfare system-
- Experience working with partnerships and across sectors-
- Knowledge of and experience working with local community resources in Northeast Edmonton-
