The Job Summary:
Key Accountabilities & Success Criteria:

- Conduct Long Term Care (LTC) resident waitlist assessments ensuring compliance with Fixing Long Term Care Act 2021 (FLTCA) and a smooth transition into LTC.
- Support advanced care planning, prioritizing residents’ preferences and end-of-life care needs.
- Facilitate access to financial resources and guide families through funding options and tax-related processes.
- Educate and assist with Power of Attorney, capacity assessments, and Substitute Decision-Making arrangements.
- Coordinate collaborative care plans with families, healthcare providers, and community partners.
- Maintain clear communication and effective relationships with community agencies, health care facilities, residents and families to ensure transparency and trust.
- Collect and utilize feedback to enhance assessment processes and care experiences.
- Provide accessible information and resources to help residents and families make informed decisions.
- Provide information to potential residents and/or families regarding the admission process.
- Act as a liaison with community agencies and partners regarding the placement and support of prospective residents.
- Review all Home and Ontario Health atHome (OHaH) files on the organization’s waitlist.
- Facilitate and review for all waitlisted residents in collaboration with the LTC Admissions Coordinator and OHaH Care Coordinators.
- Lead and coordinate all aspects of the resident admission including ensuring staff are available to complete the admissions, communicating with new admission/SDM, and communicating with clinical staff in preparation for the admission.
- Collaborate with the multi-disciplinary team to coordinate the resident’s admission.
- Coordinate follow up with resident and Substitute Decision Maker (SDM) following admission for satisfaction and feedback.
- Notify Director of Long-Term Care, Assistant Director of Long-Term Care, Registered Staff and interdisciplinary team of all pertinent information to establish a resident care plan.
- Promote an environment that encourages and supports change using change leadership theory.
- Contribute to the organizations efforts and initiatives to enhance SJHCG’s missions, vision and values by developing a culture of resident and staff safety through accountability, reliability, respect, trust and teamwork.
- Perform duties in accordance with all applicable legislative requirements (i.e. Ministry of Long-Term Care Act/Reg., Ministry of Labour, Public Health Act, etc.).
- Work in compliance with the Occupational Health and Safety Act and its regulations, reporting hazards, deficiencies, and contraventions of the Act in a timely manner, taking action to address any items.

Qualifications:

- Diploma/Degree in health related discipline (e.g. nursing, laboratory, epidemiology, and other allied health) or equivalent combination of education and experience
- Regulated Health Professional registered with a College in good standing without restriction.
- Two (2) years of recent experience in community health or similar field.
- A valid driver’s license and access to a reliable vehicle.

Skills & Abilities:

- Comprehensive knowledge of standards set out by the Ministry of Health and Ministry of Long-Term Care.
- Strong understanding of Ontario’s health care system and community resources.
- Strong computer skills, including but not limited to Point Click Care and Microsoft Office.
- Excellent interpersonal skills; able to build and maintain positive working relationship with a variety of stakeholders and work as a team player.
- Strong verbal and written communication skills; able to prepare policies and procedure and communicate regarding best practices.
- Ability to work with mínimal supervision and demonstrate sound decisions making, time management, and administrative skills.
- Strong customer service skills, with the ability to effectively assist and support clients and their families in a compassionate and professional manner.
- Ability to prioritize and work efficiently and accurately to meet deadlines in a fast-paced environment.
- Strong interpersonal skills, patience, and oral and written communication skills to effectively communicate with resident
