**MAIN FUNCTION**:The Street Outreach worker provides support to homeless individuals living on the street, in parks, ravines to address their basic needs to live a life away from homelessness and toward a more self-sufficient lifestyle, a while ultimately working towards finding safe and sustainable housing.

**DUTIES AND RESPONSIBILITIES**:

- Engage and build rapport with clients who are living on the street, in parks and ravines.
- Address clients’ basic needs and determine readiness for housing
- Create client files and input all client information within “SMIS” (our client’s database) as per our funding agreement.
- Identify types of housing needs including shelter, transitional, supportive, long-term care facilities as we work towards permanent residential settings.
- Support and accompany clients to housing and other appointments and services when appropriate
- This position will require driving agency vehicles and making scheduled stops at drop-in sites but not limited to parks and ravines to distribute essential supplies, clothing and hygiene supplies to individuals experiencing homelessness
- Assist clients in locating and utilizing community resources as support to maintain their housing
- Provide eviction prevention and intervention service to clients whose housing security is at risk
- Participate in regular case management supervision
- Submit completed case summaries to the Manager
- Maintain up to date files on each client including case notes and referrals
- Ensures that program delivery is consistent with Indigenous Culture and traditions
- Ensures that program delivery meets the City of Toronto’s Housing First Framework
- Other duties as identified by their supervisor

**SKILLS REQUIRED**:
Familiarity with housing resources in Toronto
- Ability to prioritize and manage multiple jobs effectively
- Ability to work under pressure and to meet deadlines
- Able to communicate effectively - Good oral and written communication skills
- Knowledge and ability to work with computers
- 10 years’ driving experience insurance requirement
- A valid Ontario Driver’s License and clean driving abstract is a requirement

**EDUCATION & EXPERIENCE**
- An education in social services or other related discipline or equivalent
- Several years of experience in a related organization
- Must pass Vulnerable Sector Check
- Successful experience working with a diverse population
- Knowledge of Indigenous culture and traditions is an asset
