The **RPN Manager of Clinical Practice** for the Home Support Program, under the direction of the immediate Manager, is responsible for the delivery of client centered care and service that focuses on the individual needs of the client to maintain ability to remain in home for as long as safely possible.

**Duties and Responsibilities**:

- Review referral for care and plan initial home visit/assessment as per funder contractual timeline, including all documentation for Bayshore and Funder, follow up with funder for additional service providers as required during visit.
- During visit Manager of Clinical Practice is responsible for assessing client care needs and ability to assist with personal care, developing collaborative care plan that is focused on client strengths and weaknesses, ensuring that environment is safe for both client and staff, discussions about contingency planning and review of essential policy that affect the safe delivery of care.
- Annual Clinical Management Revisits with client/family to reassess and ensure that care plan is current and accurate for client needs and continued evaluation of client environmental safety.
- Liaise with Funder as required to address any concerns about client needs or need for addition supports in the community.
- Assist with Clinical Orientation for new field staff and assist with ongoing in servicing initiatives for field staff.
- Organize and deliver community-based PPE drops to support filed staff needs and offer additional opportunity for face-to-face interaction/opportunities for education.
- Participate in Employee engagement and support visits, with addition community support as required for field staff.
- Facilitate Transfer of Skills for unregulated staff for individual care plan needs for clients with other service providers ensuring staff competency with skills required for client care.
- Participating in rotations as in-office Manager of Clinical Practice for daily escalations and Manager of Clinical Practice on-call rotations for afterhours support to address client concerns in a timely manner.
- Complete other tasks as requested.

Job Qualification
- Graduated from an accredited School of Nursing, as a Registered Practical Nurse (RPN); holds current registration with the provincial regulating body and in good standing.
- Recent experience with progressive responsibilities as a Nursing Supervisor (Preferred but not Mandatory)
- Minimum of 5 years of nursing experience

**Other Skills and Abilities**

Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format; ability to work independently and effectively lead a team; above average report writing and communication skills with both Field and Administrative employees; valid driver’s license and evidence of current CPR certification; solid knowledge of the principles, practices, and methods of community nursing and service delivery to clients as well as program development, implementation and evaluation.
