Job DescriptionMississauga-Halton Central Intake Program is the single access point for referrals for diabetes education services in the Mississauga Halton region.The Central Intake Clinician is accountable for managing the day-to-day clinical activities related to diabetes referral management. This includes:Review incoming referrals to diabetes education programs and other related chronic disease services for clinical acuity, content completion and service requestTriage and route referrals to appropriate diabetes program following criteria and guidelinesCollaborate with internal and external stakeholders to ensure ongoing evaluation and streamlining of the referral processConducting data analysis of referral patterns to identify gaps services Develop and maintain appropriate triaging and decision tools and updated policy and procedure manual to support the central intake process and operationDeveloping methods to ensure integrated care coordination between the Central Intake Program, referring physicians and diabetes education programsAssist with the adoption and utilization of the eReferral solution for diabetes referral managementDevelop targeted quality improvement strategies in collaboration with specific stakeholders to implement ongoing process improvement and adoption of the Central Intake Program, including participation in regional planning activitiesQualificationsRegistered Nurse or Registered DietitianCertified Diabetes Educator certification preferredMinimum 3 years experience in direct diabetes education and managementDemonstrated ability in clinical assessment, critical thinking and problem-solvingDemonstrated skills in collaboration, organization and change managementStrong knowledge of regional diabetes education and other chronic disease servicesUnderstanding of healthcare delivery processes, benchmarking, activity trackingUnderstanding of electronic referral solution and impact on workflow preferredAbility to work independently and set work priorities in collaboration with team membersStrong written and verbal communication skills and excellent customer service skillsAble to operate with confidence in a self directed manner, comfort with new and evolving processes, strong problem identification and resolution skillKnowledge of other languages, including French is a strong assetMust be physically able to do the jobExcellent employment and attendance record  Additional InformationHours: Monday to Friday, 8am-4pmSalary: $46.004 to $59.637 per hourDate Posted: December 1, 2025Internal Applicants Due By: December 8, 2025REF2974MHalton Healthcare is an equal opportunity employer who is committed to integrating diversity, equity and inclusion throughout our operations, policies and culture.
