As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.Please note that this position is located in Point of Access, Community.What will you do?In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plansLink patients with service providersCoordinate and monitor care plan deliveryEstablish a helping relationship with patients and their familiesBalance patient needs and choices with available resources, ensuring patients’ values and preferences are respectedWhat must you have?Membership, in good standing, with the applicable regulatory body:College of Nurses of Ontario (RN)College of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioOntario College of Social Workers and Social Service Workers (RSW)2+ years of recent experience in community health or a related fieldKnowledge of the health care delivery system and community resourcesExcellent interpersonal, communication, assessment, problem-solving, and decision-making skillsEffective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environmentEstablished ability to accurately complete required documentation, reports and formsA valid driver’s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID-19 vaccination policy. What would give you the edge?Experience working with diverse patient groups, ., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatricsCase management experience or recent related community experienceAbility to speak French or another second languageHours of work:Staff will work extended hours shifts: 8:30am – 8:30pm, 7 shifts in a two week period; 70 hours per bi-weekly pay period. Candidates must be flexible to adjust to shifts within the hours of operation. The successful incumbent will rotate through different extended hours shifts as required.Position location and travel:Regular travel within the geographical region of Ontario Health atHome Central East may be required.
