This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require in-person home visits with vulnerable community members. Key Responsibilities: Complete initial home-based physiotherapy assessments within 24 hours of referral Provide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression Develop, implement, and adjust individualized treatment plans in alignment with client goals Educate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment use Identify and address fall risks, environmental barriers, and safety issues in the home Collaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated care Complete documentation and charting accurately and in a timely manner according to community-care standards Maintain ongoing communication with the office regarding availability, scheduling updates, and urgent visit capacity Uphold The Care Company’s commitment to safe, ethical, and compassionate community careRequirementsScheduling: Flexible part-time schedule (Monday–Friday) Occasional Saturday/Sunday availability based on client needs Must be able to complete an initial in-person home assessment within 24 hours of receiving a visit request Clients are located across Toronto and the Greater Toronto Area, including but not limited to: Downtown Toronto, Midtown, East/West Toronto North York, Richmond Hill, Markham Mississauga, Brampton, Vaughan, Woodbridge Pickering, Ajax, Whitby, Oshawa, Bowmanville and select areas of Durham region Ability to independently manage scheduling, respond promptly to referrals, and coordinate visit times directly with clients Travel may be required between client homesQualification and Experience Requirements: Home and community care experience is a MUST. Applicants without community experience will not be considered.
