Joining us at East Kootenay Primary Care Network (PCN), you’ll have a pivotal role providing social work services to unattached clients in the Invermere Health Centre. PCN Social Workers help individuals manage a variety of emotional and social concerns that impact their health and wellbeing. They can assist with housing and food security, mental health concerns, identity needs, and crisis and safety needs. They support connection of individuals with appropriate community services, assist with access to benefit services, and coordinate with individuals’ healthcare teams. Days of work: Monday to Friday; 08:30 to 16:30. This is a relief, full-time opportunity until return of incumbent (unknown). Once hired into a relief role, employees are eligible to apply for internal postings including permanent positions. In a PCN, family physicians, other primary care providers, and allied health care providers in a geographic area work together to meet the health needs of their local community. East Kootenay PCN clinical teams are patient‐focused, coordinated teams and include physicians, nurse practitioners, nurses, allied health providers (social workers, occupational therapists, physiotherapists, mental wellness clinicians, dietitians, pharmacist, respiratory therapists) and Indigenous health coordinators actively working together to address the primary care needs of East Kootenay residents.
