Detailed Overview Provides comprehensive care management and services to complex clients and adults with acquired brain injuries within a multi-disciplinary team setting; works with clients/families to address their health, psycho-emotional, social, environmental and functional needs to maximize the client's capabilities, community participation, recovery from or adaptation to illness or disability and to decrease the potential for admission to hospital or residential care; establishes and maintains relationships by collaborating and facilitating access to a wide range of services to meet individual client needs. Responsibilities Carries out health, psycho-emotional, social and functional/environmental assessments to identify client strengths, risks, needs and goals in collaboration with client/family/health care team; develops a care plan to meet these goals and promote stability in client/family situation; monitors and evaluates effectiveness of the care plan on an ongoing basis; implements care plan adaptations, as necessary. Maintains responsibility and accountability for approving and facilitating access to regionally funded community health resources such as home support services, adult day programs and residential care as part of the overall care plan. Uses knowledge and skill in area of professional practice, clinical assessment and care management to independently manage a complex client case load; participates in education programs and evaluates and develops own competencies and abilities to maintain and improve professional standards. Develops and monitors individual client and funded contract support services for clients; evaluates effectiveness and quality of services provided to clients by conducting on-site inspections and facilitating follow-up action, as per established policies and procedures. Facilitates client's transitions through a variety of settings within the healthcare system; by providing leadership and collaborating in interdisciplinary client/family conferences. Advocates on behalf of clients and families to access system information and resources; promotes client''s right to autonomy, dignity and privacy; advocates for system/program change that will broaden services and enhance the systems capacity to support clients/families. Develops and facilitates psycho-educational and support groups for individuals with acquired brain injury, families and their caregivers; provides training/educational seminars/workshops for professionals/caregivers/ clients. Mentors and orients new staff, students and other health professionals as requested; represents Acquired Brain Injury Services by providing public information and education. Maintains complete and accurate clinical records and/or files and ensures documentation is shared between clinical professionals and other community service providers; monitors and prepares individual and summary reports. Participates in local, regional and external committees and working groups to strengthen professional practice and team development; provides input into operational issues. Provides instruction, feedback and supervision to students as assigned. Performs other related duties as assigned. Qualifications Education and Experience Bachelor's of Social work from an approved school of Social Work. Two years' recent experience working with complex acquired brain injury clients with chronic and acute illness, including experience in care co-ordination and service planning across community and/or residential based healthcare system. Current full registration with the BC College of Social Workers. Valid BC driver's license and access to personal vehicle for business related purposes. Skills and Abilities Knowledge of the broad health system, community resources and volunteer agencies, and their roles and responsibilities in providing a continuum of care. Knowledge of disability issues and the needs of adults living with permanent injury and illness. Knowledge of relevant legislation, standards and policies. Demonstrated ability to apply knowledge of theory and practice to the case management process. Demonstrated ability to conduct psychosocial assessments. Demonstrated ability to deal with others effectively. Demonstrated ability to work as a member of an interdisciplinary team in care planning for complex client situations. Demonstrated ability to communicate effectively both verbally and in writing with clients, families, other members of the health care team and community agencies. Demonstrated ability to independently manage and prioritize caseload/workload and make decisions regarding interventions and accessing subsidized resources. Ability to operate related equipment including applicable software applications Physical ability to carry out the duties of the position.
