As a Care Designer you are responsible for the day to day operations related to driving business results through client experience and caregiver management.You will drive growth through the following main duties and responsibilities:   Consult Conversion:Conducts Caring Consults, assesses Clients with respect to physical, emotional andenvironmental needs and advises on Client care; achieves conversion goal of 90%Drives business interest by engaging in relationships with community leaders andprofessional organizations to establish long term relationships that will translate intoword of mouth and professional referralsDevelops, promotes and ensures high quality, innovative customer serviceActs as advisor in Caring Consult to ensure that Client and Client contacts have allinformation required Client Care Delivery and Experience:Attends all First Visits with Caregivers to ensure client needs and care plans areexecuted as per the Service Agreement; sets and role models expectationsConducts supervisory visits to ensure Caregiver fit and quality of care; delivering apremium client experience impacting Net Promoter Scores of 9 or higherDevelops appropriate and individualized care plans and updates as requiredEducates clients on self-care and evaluates client response to care providedMonitors risk issues and analyzes trends, adapts care as neededCoordinates care, implementing clinical solutions, and evaluates client outcomesbased on client goals People Leadership:Drives results through leading your caregiving team by setting clear goals, assigningtasks, coaching in the moment and providing continuing educationDelivers on our Brand Promise by leading the client experience, ensuring ourCaregivers are doing whatever it takes to bring peace of mind to our clients and theirfamiliesAccountable for staff hiring, training and performance, ensuring Caregiver availabilitymeets business needs and that there is compliance to all systemsManages the education function to support a continuous learning environment andstaff utilization of learning opportunities.This includes Discovery Day and monthly in-service programsConducts First and Supervisory visits to evaluate Caregiver performance and jobsatisfaction, impacting the employee experience resulting in a Net Promoter Score of9 or higher Scheduling Collaboration:Liaises with the scheduling team to communicate changes in client requirements,Caregiver assignmentsResponsible for ensuring client coverage is secured, particularly due to urgent care,caregiver cancellations and limited availability; assists with scheduling as neededResponsible to ensure AMS (Attendance Monitoring Investigations- ALAYACARE)percentages remain below 5% for total staff; manages performance of CaregiverResponsible for data integrity and completion (Caregiver & Client files) in thescheduling software Administration:Maintains excellent documentation practices, specifically, complete and accurate careplans, employee files (skills & qualifications), client files, including pay and billinginformationReviews and updates client and staff records quarterly to ensure 100% accuracy andcompletenessDirects the maintenance of client & personnel records to meet employee,organization and statutory requirements and entitlementsConducts record audits on a quarterly basis to ensure accuracy and up to dateinformationEnsures compliance with national/provincial legislative requirements in areas such ashealth, employment, occupational health & safetyResponsible for approving all visits and premiums in preparation for each payroll andbilling cycle   Availability: Flexibility is required (availability 7 days per week)Schedule provided 2 months in advance as per business needsPart time ( primarily evenings and weekends)On-call evenings, holidays and weekends Business Relationships:Reports to the Franchise PartnerLeads the Caregiving team and Care Ambassador teamWorks closely in partnership with Care Services Center Qualifications:Requirements:Registered Practical Nurse or Registered Nurse, licensed to practice in theProvince of OntarioCPR and First Aid CertificationCar and valid driver’s license Experience:Strong computer skills, ideally proficient in G-Suite: Gmail, drive, docs, calendar as well as electronic medical documentation (Alayacare Experience an asset)Exceptional written and verbal communication skills in the English languageIntentional organization, planning, time management and multitasking skillsDemonstrated experience setting and achieving goalsCustomer service and leadership background considered an assetBackground or Criminal Record Check with a Vulnerable Sector Search(updated every 5 years for continued employment)Negative TB test (updated every 5 years for continued employment)     MISSION STATEMENT Nurse Next Door is a team that is dedicated to delivering responsive, caring, and high-quality home health services to our community.  Our core purpose is: Making Lives Better.  It is why we started the company, and it is a reminder of why we come to work every day.  Our core Values are apparent in everything we do: Admire our People WOW our customers Find a Better way Being passionate about making a difference   PHILOSOPHY Nurse Next Door believes that each individual, regardless of age, race, color, religion, sex, marital status, national origin, handicap, or source of payment, is entitled to maximize his/her optimum level of health status towards more complete physical, mental and social wellbeing. To provide relevant educational opportunities to Nurse Next Door personnel so that knowledge and caregiver expertise is enhanced for the benefit of the clients they serve.
