About the Opportunity Reporting to the Health Services Manager (HSM) the Frailty Coordinator provides leadership and support in the development, implementation, evaluation and ongoing support for of the Early Mobility Programs and other Frailty Network initiatives as identified by the Nova Scotia Health (NSH) Frailty Network and zone leadership. The Frailty Coordinator will support improved awareness of Frailty through identification, management and prevention strategies within all areas of the organization while working within the Mission, Vision and Values of NSH. Specific focus will be leading inpatient mobility teams throughout the zone, education on the use of the Clinical Frailty Scale (CFS) and collaboration with the interdisciplinary health care teams in the implementation and evaluation of this work. The Frailty Coordinator is responsible to oversee the coordination, training, implementation, and ongoing development of the mobility program members as well as provide education related to care of hospitalized older adults to frontline staff and clinicians. The Frailty Leader may provide clinical interventions such as assisting with care plan development or mobilizing patients as per their professional license or registration.About You We would love to hear from you if you have the following: Bachelor’s Degree in a regulated health care discipline – Occupational Therapist, Physiotherapist or Nursing or Other Regulated Health Profession Maintain current license/registration with the Nova Scotia College and maintains eligibility for membership in the Canadian Association if applicable Three years relevant experience in healthcare delivery, quality improvement, program development, design, implementation and evaluation Familiarity with health records and EMR Demonstrated ability to work independently and collaboratively to successfully implement quality initiatives in a busy health care setting Computer proficiency in Microsoft Suite or equivalent, with emphasis on PowerPoint, word processing, spreadsheets, graphs/charts and data management Excellent research, data collection, report writing, facilitating, time management, organizational and problem-solving skills Strong understanding of Frailty and a working knowledge of the Clinical Frailty Scale (CFS) Passion for improving care for older adults based on current best practices and published evidence May include weekend shifts Competencies in other languages an asset, French preferred Please ensure your resume is up to date and includes all relevant education, experience, training, and certifications.Hours Full Time Long Assignment; 75 Hours Bi-weekly Approximately 18 month term; Dates are subject to change Compensation and Benefits $42.19 - $53.97 Hourly Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
