POSITION SUMMARY:St. Joseph’s Home Care (SJHC) is dedicated to providing quality, compassionate, and client centered care to individuals in their homes and/or alternative settings through visiting, shift or the Integrated Comprehensive Care programs. Registered Practical Nurses enhance the life of their clients by providing a broad range of care needs within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. Through their work, Registered Practical Nurses contribute to the healing and independence of their clients and promote the continual improvement of the overall health of the community.CORE DUTIES AND RESPONSIBILITIESUtilizes the nursing process effectively, in order to provide high quality Wound and Ostomy Care for clients and/or caregivers. Collaborates with the interdisciplinary health team to provide cost-effective, high-quality Wound Care, using Best Practices.Applies current knowledge and research of Wound Care Protocol to practice.Creates, in consultation with the physician, CCAC Case Manager, collaborate with ICCP coordinators and Primary Nurse, a plan of care for patients requiring wound care in the community. Initiates plan of nursing care in consultation with the client and/or caregiver and modifies plan of care on an ongoing basis. Promotes the independence of clients and caregivers considering their needs, strengths and weaknesses.Utilizes available resources, facilitates clients/caregivers' problem solving, participation in decision making, and access to appropriate resources.Participate in the evaluation and refinement of the Wound Care Protocol and overall program.
